Certification Statements for State and Local Contract Bids
This document is a Certification Statements form issued by the State of Ohio for bidders seeking preference designations on state and local contract opportunities. The form requires bidders to complete appropriate certifications to qualify for specific bid preferences, including the Domestic Source End Products preference (Buy American), Ohio preference (Buy Ohio), and Veteran Friendly Business Enterprise (VBE) preference. No specific products, services, quantities, response dates, or contract terms are identified in this document, as it serves as a standardized certification template applicable to multiple contract opportunities. The form emphasizes that bidders must complete the appropriate certification section to receive any preference, with the state reserving the right to clarify information during the evaluation process.
The document establishes strict compliance requirements, warning that bidders who intentionally submit false or misleading information to receive a bid preference will be immediately disqualified and may be subject to legal action up to and including debarment. Additional certification requirements include confirmation of E.D.G.E business designation status and verification that the bidder qualifies as a business entity under O.R.C Section 145.037. The Domestic Preference section allows bidders to identify products manufactured in the United States, Canada, or Mexico, or those with at least 50 percent of component costs produced within the United States. The Ohio Preference section requires bidders to demonstrate either that products are manufactured in Ohio or that the bidder maintains significant economic presence within Ohio, including tax payment, Secretary of State registration, and minimum employee thresholds. Border state provisions apply to businesses in Kentucky, Michigan, New York, Pennsylvania, and Indiana.
