About this file
This is a Certificate of Procurement Integrity form applicable to multiple state and local contract opportunities across several jurisdictions, including the Town of Berlin, Connecticut; the City of Conroe, Texas; the City of Newark, New Jersey; and the Transit Authority of Lexington-Fayette Urban County Government in Kentucky. The certificate pertains to diverse service procurements including architectural and engineering services for park renovation (Berlin, CT), professional consulting services for survey, geotechnical, and engineering work (Conroe, TX), water purification chemicals (Newark, NJ), and landscaping services (Lextran, KY). The Berlin project involves design and construction administration for two-phase park renovations at DDB Memorial Park, with Phase 1 addressing pool-to-splash pad conversion and parking expansion, and Phase 2 involving tennis-to-pickleball court conversion and playscape installation. The Lextran landscaping services solicitation has a proposal deadline of 2:00 pm Eastern Time on Wednesday, June 3, 2026, with questions due by May 1, 2026. The Newark water purification chemical bid receives submissions until 10:15 am on Thursday, February 19, 2026. The Conroe City solicitation seeks prequalified firms for multiple professional services categories. The Berlin project timeline targets late summer/early fall 2026 commencement upon permitting completion.
The Certificate of Procurement Integrity form requires that the certifying officer or employee responsible for proposal preparation attest compliance with the Office of Federal Procurement Policy Act (41 U.S.C. 423) and Federal Acquisition Regulations provisions concerning procurement integrity violations. All participating officers, employees, agents, representatives, and consultants must certify familiarity with and commitment to compliance with these requirements and agree to report immediately any suspected violations. False or fraudulent certification may subject the certifier to prosecution under 18 U.S.C. Section 1001. The form does not establish specific pricing terms, set-asides, or funding amounts within the certificate itself; however, the related opportunities indicate municipal funding sources and estimated project ranges such as $50,000 to $250,000 for splash pad restoration projects. All jurisdictions explicitly confirm equal opportunity employer status and non-discrimination policies. Proposals must be submitted via electronic portals designated by each respective agency, and late submissions will not be accepted.
