The six required data items are: (1) Data Conversion Plan (A001)—a one-time submission due within 30 days of contract signature, containing a detailed project timeline, data maps for bibliographic/patron/circulation/serials/authority/usage data, data validation methodology, and Government review/acceptance process; (2) Data Migration Progress Report (A002)—weekly reports beginning seven days after migration phase start and continuing every Monday, including migration statistics, error summaries, and updated timelines; (3) Data Migration Exception Log (A003)—weekly cumulative logs delivered concurrently with progress reports, detailing failed records, error messages, and corrective actions in CSV or XLSX format; (4) Post-Migration Validation Report (A004)—weekly reports tracking cumulative failed migration records with error details and proposed corrections; (5) UAT Issue Resolution Report (A005)—daily reports during User Acceptance Testing beginning one day after UAT start, tracking issue reporters, descriptions, vendor analysis, and resolution status via formal report or live issue tracker (Jira/Trello); and (6) Final Implementation Report (A006)—a one-time submission due five days after the ILS "Go-Live" date, confirming completion and acceptance of all prior phases and full data migration. All reports are delivered via email to library staff members.

16. REMARKS
Content shall be as specified in SOW para 2 – sub-para 3a and DI-IPSC-81427.
A plan to be distributed ONE TIME (see Block 10), no later than 30 days after the contract is signed
(see block 12).
The plan must include, at a minimum:
a. A detailed project timeline with key milestones for all project phases.
b. Data maps for bibliographic, patron, circulation, serials, authority, and usage statistics data.
c. The proposed methodology for data validation and testing.
d. A description of the formal process for Government review, validation, and final acceptance.
e. All formal reports will be delivered by email to all members of the library staff as set out at the beginning of the process.

The first report will be delivered seven days after the beginning of the data migration phase (see block 12), with each subsequent report delivered every Monday (see block 13).
The report must include, at a minimum:
a. Must include migration statistics (attempted, success, fail), summary of errors, and updated timeline.
b. All formal reports will be delivered by email to all members of the library staff as set out at the beginning of the process.
c. Subsequent reports after the first shall be delivered every Monday thereafter.
