About this file

This document contains additional BAA (Broad Agency Announcement) representations and certifications that contractors must complete, including FAR 52.209-7 regarding responsibility matters and DFAR provisions. The form requires contractors to disclose whether they have current active federal contracts and grants exceeding $10 million, and if so, to certify information in FAPIIS regarding any criminal convictions, civil proceedings, or administrative findings within the past five years that resulted in monetary penalties or liabilities.

The document includes provisions for identifying technical data rights and software restrictions (DFAR 252.227-7017), disclosure of previously delivered technical data/software to the government (252.227-7028), and representation regarding transportation by sea (252.247-7022). Contractors must complete a table listing company principals and their positions. The form requires detailed disclosures about proceedings that resulted in fines over $5,000, restitution over $100,000, or criminal convictions related to federal contracts, and mandates reporting this information in the Federal Awardee Performance and Integrity Information System (FAPIIS).
