About this file
This Request for Proposals (RFP) issued by NorthWest Senior & Disability Services in Salem, Oregon solicits qualified flooring contractors for carpet tile installation and repair services at the Salem Annex Building, Suite 190. The project scope includes installation of approximately 7,100 square feet of Patcraft Mid Century Mad Collection Orbital carpet tiles (Style 10379, color Doo Wop, 24" x 24" format) with no product substitutions permitted, repair of approximately 120 carpet tiles in an adjacent hallway, and installation of 4-inch commercial-grade cove base. The RFP was issued on February 6, 2026, with a proposal deadline of February 27, 2026 at close of business. An optional site visit is scheduled for Thursday, February 19, 2026 from 9:00 am to 12:00 pm at the project location, with attendance strongly encouraged as it will be the only available site access day. Contractors must plan for approximately 30-45 minutes on site during the visit window.
Proposals must include company information, relevant project experience with references, detailed cost breakdowns by labor, materials, adhesive, cove base, demolition, and contingent items, and two pricing options—one including carpet tiles and one assuming NWSDS provides the tiles. All work must comply with Patcraft installation guidelines, utilize low-VOC adhesives suitable for occupied commercial environments, and maintain daily work hours of 8:00 am to 4:30 pm with sequencing to minimize business disruption. Contractors must provide proof of current general liability and workers' compensation insurance, Oregon CCB license numbers, and material and workmanship warranties with coverage details. Evaluation criteria prioritize RFP compliance, proposal quality and completeness, demonstrated experience and qualifications, proposed schedule, licensing and insurance documentation, responsiveness, and cost clarity, with cost considered alongside qualifications rather than as the sole determining factor. The Owner reserves the right to reject proposals, award all or part of the work, or phase the project as operational needs require.
