About this file

This is a California Civil Rights Laws Attachment form issued by the California Department of General Services, Office of Legal Services, required for all contracts with state agencies valued at $100,000 or above. The attachment serves as a certification requirement for bidders and proposers entering into or renewing state contracts executed or renewed after January 1, 2017. Contractors must certify compliance with two key California civil rights statutes: the Unruh Civil Rights Act (California Civil Code Section 51) and the Fair Employment and Housing Act (California Government Code Section 12960). Additionally, contractors must certify that any internal policies regarding sovereign nations or peoples recognized by the United States government are not applied in violation of these two civil rights laws.

The certification must be executed under penalty of perjury by an authorized representative of the proposer or bidder firm and submitted at the time of bid or proposal submission or at contract renewal. Required documentation includes the contractor's printed firm name, federal identification number, the name and title of the signing officer, the county and state where executed, and the date of execution. This form represents a mandatory compliance requirement established by California Public Contract Code Section 2010 and applies uniformly to all qualifying state contracts regardless of scope or subject matter.
