Statement of Work

Period of Performance: 30 May 2025 - 29 May 2030

BASE PLUS FOUR OPTION YEARS

1. Scope

1.1. The contractor shall provide coverall rental and laundry services of vendor provided coveralls to the Fleet Readiness Center (FRC) East, Marine Corps Air Station, Cherry Point, North Carolina. Services shall include weekly pick-up of soiled coveralls at the identified locations, and delivery of the laundered items to the same location within seven (7) days.

1.2. The contractor shall provide all services to launder vendor provided coveralls to include equipment, supplies, and supervision of employees and transportation for pick-up and delivery of items.

2. Requirements

2.1. The Contractor shall pick up soiled coveralls at the designated location once per week and have them laundered as specified. The initial pick-up shall be made upon mutual agreement between the FRC East Representative and the Contractor. The initial delivery may be made any day of the week, Monday through Thursday, between the hours of 7:30 AM and 2:00 PM. However, once a day is established, the same delivery day shall be maintained every week, except on government observed holidays.

2.2. Federal Holidays: Contractor is not required to provide services on the observed Federal holiday. The following are the Federal Holidays observed by this installation:

2.3. The contractor shall complete the laundering services and return the coveralls back to the location within seven (7) days. Heavily contaminated coveralls shall be identified by the shop artisans, stored and picked up separately, and laundered separately to prevent contamination of the general lot of coveralls during the washing process. The contractor shall deliver the laundered coveralls back to the delivery point and pick up the soiled coveralls that have accumulated during the week. This shall continue for the term of the contract for the designated delivery point to allow maintenance of an inventory at the delivery point.

2.4. The coveralls shall be washed, dried, folded and separated by size. The contractor shall ensure that the coveralls are wrapped in paper or plastic packaging so as to prevent the items from being soiled in transit. The contractor is not permitted to use hangers to return the coveralls to the location.

2.5. The coveralls shall be delivered to the specified location. The deliveries shall be of an equal quantity and size as that of the soiled pick up, a one for one exchange in size and quantity.

2.6. The coveralls will be inventoried at the delivery point at the time of pick-up and delivery by an authorized government representative and the contractor's representative. The contractor shall bear the responsibility of replacing any coveralls that are lost or damaged while in his possession. The contractor will also inform the FRC point of contact of any damage or missing coveralls on a monthly basis.

2.7. The contractor shall bill at least monthly, but no more frequently than weekly. The Contractor shall provide pre-printed tickets for the verification and signature of items picked up and delivered. Signature spots shall be on this form for both the Government Site Representative and Contractor Representative. All other areas of the format can be in accordance with industry standards. Counts of all items to be laundered shall be identified on the ticket for pick-up and delivery.

3.0 Contaminants.

The Contractor is hereby advised the soiled coveralls may be contaminated with minute quantities of the following substances:

4.0 Laundry Performance Requirements

4.1 All services shall be performed in a manner that is consistent with normal commercial practice for commercial laundry operations.

5.0 NAVY BLUE COVERALLS

5.1. The contractor shall provide NAVY BLUE coveralls to one (1) locations. The estimated quantity for each pick-up and delivery of the NAVY BLUE coveralls shall be 230 pairs of coveralls on hand (see below for sizes and locations).

5.2. The coveralls shall be NAVY BLUE in color in order to distinguish them from the other coveralls being laundered. The coveralls shall be made of cotton and the type commonly furnished by commercial laundries

7.0 Contractor Performance Requirements:

7.1 Safety, Environmental, Quality and Security Requirements: All personnel who enter or work in assigned spaces of this facility are required to comply with all federal, state, local, and site specific safety, environmental, quality and security requirements. FRC East has adopted and implemented additional safety programs to improve safety, quality, environmental, and security measures. Any individual found to not comply with this effort will be asked to immediately halt work.

7.2 Supervisors, managers, and employees are to report issues of non-conformance through the proper chain of command. Any individual or entity not willing to comply will state such and shall not be authorized to enter the facility. FRC East may take actions that result in removal of individuals or entities from this facility due to issues of non-compliance with stated instructions.

7.3 Documented proof of all required certification training/licensing listed in the contract shall be available upon request by their Contracting Officer Representative or any auditor.

7.4 All personnel working on site at the FRC East under this contract shall document their receipt and review of the information listed in reference (a) annually.

7.5 All contractors performing work on FRC East behalf and who have 10 employees at any time during the past calendar year, whose employees worked 1,000 or more hours in any quarter shall furnish FRC East their Total Case Incidence Rate for recordable nonfatal injuries and illnesses and their Days Away from Restricted work activity, and/or Job Transfer Rate for the past three years.

7.6 Foreign Object (FO) is defined as any article or substance alien to the aircraft or assembly which is allowed to invade the product. Foreign object damage (FOD) is the damage that occurs due to these FOs. All FRC East work sites shall be maintained in such a manner as to prevent FOD to aircraft and/or aircraft components. Work sites shall be kept clean at all times. All debris, scrap material, tools, and equipment will be cleared from the work site as work progresses. At no time shall hoses, power cords, materials, etc. be permitted to create tripping hazards in areas of the work site.

7.7 In those cases where a contractor supervisor determines that solving a safety or health problem is beyond their control, but within the control of the FRC East, the contractor shall notify the appropriate point of contact identified in the contract.

7.8 All contractor employees performing work on site at FRC East shall immediately report any safety, security, or environmental violation to their contractor supervisor, as well as the cognizant FRC East Safety/Security /Environmental Office provided in reference (a). The initial FRC East notification can be made via phone or e-mail and should include as many applicable details as possible (date, time, identification numbers, tags, company, etc.).

This initial notification shall be made as soon as possible. A safety incident will require the contractor to complete and submit an incident memo to the appropriate point of contact identified in the contract, with a copy to FRC East Safety Office within twenty-four hours of accident/incident). This incident memo will include the full name of the person involved in the incident, age, sex, job title, the name of the employing company and the contract number and title. In addition, this memo will include the severity of the illness or injury, indirect cause(s) or the accident and whether personal protective equipment was available and used.

7.9 All contractors and sub-contractors working within FRC East shall develop and operate effective safety and health programs.

7.10 Contractors are required to wear personal protective equipment (PPE) consisting of hearing protection, eye protection, and safety shoes. PPE will not be provided by the facility. Contractor representatives without the appropriate PPE will not be allowed access to the facility.

7.11 Work being performed is in critical areas, therefore constant control of tools and materials are required at all times. Tools and hardware will be controlled at all times to prevent migration out of the work site. Lost tools shall be reported to the tool control manager at 252-464-9741. Tools found unattended will be confiscated and reported to the contracting officer.

WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (JAN 2023)

(a) As used in this clause- Definitions.

"Department of Defense Activity Address Code (DoDAAC)" is a six position code that uniquely identifies a unit, activity, or organization.

"Document type" means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

"Local processing office (LPO)" is the office responsible for payment certification when payment certification is done external to the entitlement system.

"Payment request" and "receiving report" are defined in the clause at 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(b) The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as Electronic invoicing.

(d) . (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within [insert the period of time within which 1 day the Contracting Officer may exercise the option]; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least days[ ] before the contract expires. The preliminary notice does not commit the 1 60days unless a different number of days is inserted Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed (months) (years).60 months
