Statement of Work Summary: FR 86 Base Repair and Resurfacing Project

This Statement of Work outlines the Monongahela National Forest's requirements for repair and resurfacing of Forest Road 86. The scope encompasses milling or sawing of existing pavement, excavation of unsuitable material from failed areas, repair of base failure zones using rock and separation fabric to create French drains, placement and compaction of aggregate base stone, and placement of Hot Mix Asphalt (HMA) base material to restore original road grade. Upon completion of base repairs, HMA leveling and wearing courses will be applied throughout the project area. Additionally, shoulders will be scarified, topsoil and vegetation removed, and aggregate placed and compacted to create one-foot shoulders. Milled HMA material will be transported to FR 101 and placed in approximately three-inch-deep compacted lifts across the full road width as designated by the Contracting Officer Representative (COR). The contractor is responsible for all supervision, labor, equipment, materials, quality control, and incidental costs.

The contractor must comply with contract drawings, detailed specifications, industry standards, and applicable sections of the WVDOH 2023 Standard Specifications for Construction of Roads and Bridges. Critical contractor requirements include submitting an approved WVDOH Job Mix Formula for HMA materials, with production from a WVDOH Certified Asphalt Plant, recognizing that hauls exceeding three hours risk temperature degradation and rejection. The contractor must also submit for COR approval seven days prior to work commencement: a traffic control plan, spill prevention plan, erosion and sediment control plan, and quality control plan that includes testing protocols for HMA products, stone, and compaction with weekly reporting to the COR. The project was initiated due to extensive flood damage to FR 86 from 2015 to present and heavy equipment use, with repairs intended to restore safe and functional access for Williams River area recreation and commuting activities.

STATEMENT OF WORK

Project FR 86 Base Repair and Resurfacing Project

Scope of Work The Monongahela National Forest requests repair of base failures, pavement failures and resurfacing of Forest Road

86. Work includes milling or sawing of pavement and excavation of unsuitable material from failed areas as designated in the plans. Repairing base failure areas by placing rock and separation fabric to create French drains as needed, placing and compacting aggregate base stone, and placing HMA base material to bring the repair area back to original road grade. Once base failure repairs are complete, a combination of HMA leveling, and wearing courses will be placed throughout the entire project area. Shoulders will be scarified, topsoil and vegetation removed, and aggregate placed and compacted to create 1’ shoulders. Milled HMA material will be hauled to FR 101 and placed in approximately 3-inch-deep compacted lifts, across the full road width in areas as designated by the COR.

The Contractor shall provide all supervision, labor, equipment, materials, quality control, and incidentals for the completion of the project.

The contractor shall perform all work in accordance with the contract drawings, detailed scope of work, industry standards, and the applicable sections of the WVDOH 2023 Standard Specifications for Construction of Roads and Bridges, Supplemental Project Specifications (SPS)

Background FR 86 has received extensive damage from flood occurrences from 2015 to date and from the excessive use of heavy equipment and hauling to facilitate the repairs associated with this damage. The single lane paved roadway is integral to users of the Williams River area for camping, fishing, hunting, site-seeing, and commuting. The existing condition of the roadway makes such use difficult and dangerous. It is anticipated that the repairs associated with this contract will make the use of the road safe, comfortable and enjoyable to Forest user for years to come.

Contractor Requirements

1. Contractor shall submit an approved WVDOH Job Mix Formula for each HMA material to be utilized on the project site to the COR for approval on the project and shall produce and place the approved mix in accordance with the associated requirements, project plans and project specifications. Repair specifications require Hot Mix Asphalt (HMA) to be produced at a WVDOH Certified Asphalt Plant.

HMA quality is time and temperature sensitive. Hauls longer than approximately three hours risk the mix cooling below acceptable placement temperatures, leading to rejection at the site; a two-hour haul is ideal.

2. The contractor shall submit a traffic control plan to the COR for approval 7 days prior to starting work.

3. The contractor shall submit a spill prevention plan to the COR for approval 7 days prior to starting work.

4. The contractor shall submit an Erosion and Sediment control plan to the COR for approval 7 days prior to starting work.

5. The contractor shall submit a quality control plan to the COR for approval 7 days prior to starting work. As a minimum, the plan should include testing data for HMA products, stone, and compaction, methods to be utilized, frequency of testing, method of recording and reporting to the FS. Results of all QC testing shall be submitted weekly to the COR (at a minimum).

6. Other submittals as required by applicable contract documents shall be submitted to the COR a minimum of 7 days prior to starting work associated with those items.

7. All work shall be performed in accordance with project plans and specifications, and the WVDOH 2023 Standard Specifications for Construction of Roads and Bridges, Supplemental Project Specifications (SPS)
