About this file

This is a Proposer Information and Certification Sheet for an Oregon state contract opportunity (BID # S-91400-00017528) for Language & Accessibility Services. The document serves as the certification and authorization form that proposers must complete and submit as part of their proposal response. The form requires proposers to provide their legal entity information, including business name, address, state of incorporation, entity type, and primary contact details, along with their Oregon Business Registry Number if applicable. Proposers must certify that they understand and accept all RFP requirements, acknowledge receipt of any addenda, and commit to performing the scope of work and performance standards outlined in the final negotiated contract. The proposal constitutes a firm offer valid for 90 days following the closing date or until a date specified in section 6.3 of the RFP.

The certification sheet requires the authorized representative to attest to multiple compliance and eligibility criteria, including current tax law compliance with Oregon statutes, non-discrimination in employment and subcontracting practices, and possession of written policies preventing sexual harassment and discrimination as required by ORS 279A.112 for contracts anticipated to exceed $150,000. Proposers must also certify that they and their employees, agents, and subcontractors are not listed on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management, and that no actual or potential conflicts of interest exist between the proposer and the State. The form requires certification that all proposal contents are truthful, accurate, and prepared independently without collusion, and acknowledges that any false statements or misrepresentations may constitute a false claim under the Oregon False Claims Act with associated liabilities and penalties.
