About this file

This is an Indian Economic Enterprise (IEE) Representation Form issued by the Indian Health Service's Division of Acquisition Policy for use under the Buy Indian Act. The form serves as a self-certification document that must be completed by offerors to verify they meet the definition of an Indian Economic Enterprise in accordance with 25 U.S.C. 47.

The form requires businesses to maintain IEE eligibility throughout three key periods: when making an offer, at contract award, and during the full contract performance period. Contractors must notify the Contracting Officer immediately if they lose eligibility. The form requires specific details including the name of the 51% owner of the IEE firm, business name, UEI number, name of Federally Recognized Tribal Entity, and certification signatures. The form emphasizes that false representations are punishable under 18 U.S.C. 1001 and that successful offerors must be registered in the System for Award Management (SAM).
