About this file
The document is an Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form, designed to self-certify an offeror's status as an Indian Economic Enterprise (IEE) for federal contract opportunities. The form requires potential contractors to affirm their eligibility under the Buy Indian Act (25 U.S.C. 47) at three critical stages: when making an offer, at contract award, and throughout the contract performance period. Contractors must maintain their IEE status and provide immediate written notification to the Contracting Officer if they no longer meet eligibility requirements. The form emphasizes that false or misleading information is a violation of law, punishable under federal statutes, and successful offerors must also be registered in the System of Award Management (SAM).
The form provides spaces for key information including the name of the IEE firm owner, name of the federally recognized tribal entity, business name, and UEI number, with checkboxes to indicate whether the offeror meets the definition of an Indian Economic Enterprise. Individual Contracting Officers may require documentation of eligibility before awarding set-aside or sole source contracts, and awards may be subject to protest if the offeror's eligibility is questioned. The document is part of a Sources Sought notice for a Core Switches procurement by the Indian Health Service, with responses due by 3:00 PM CST on August 11, 2025, to be submitted via email to the Contract Specialist.
