About this file
This is a Buy American Certificate form (FAR 52.225-2, October 2022) that requires offerors to certify the domestic or foreign origin of end products being supplied under the solicitation for a Fargo-based exhaust ophthalmology chair for the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 23.
The certificate mandates that offerors certify each end product as either domestic or foreign, with domestic end products containing critical components to be separately identified by line item number. Offerors must list all foreign end products manufactured in the United States that do not qualify as domestic, and for non-iron/steel foreign products, indicate whether they exceed 55 percent domestic content (with certain exceptions for COTS items). The form includes sections for offeror signature, printed name, and position, and establishes that government evaluation will proceed in accordance with FAR Part 25 policies and procedures. This is a template form requiring completion and submission as part of the offer.
