About this file

This is a defect tracking and quality assurance report presenting bug counts and escape rates for three software programs during the period from June 2025 to April 2026.

The report tracks defects across five severity levels (Sev-1 through Sev-5) plus unadjudicated and unassigned categories for three programs: JOMIS, DHMSM, and EIDS. JOMIS reported 1,076 total defects with an 84% escape rate, including 85 Severity-1 issues and 185 Severity-2 issues. DHMSM reported 1,564 total defects with a 34% escape rate, comprising 17 Severity-1 issues and 176 Severity-2 issues. EIDS reported only 2 total defects with partial coverage due to not utilizing DT&E systems for over-the-shoulder testing. The escape rate calculation is specifically scoped to Severity 1 and 2 issues detected at DT&E and is tracked based on detection source (vendor to DT&E) rather than by release cycle. The report notes that unadjudicated bugs are initially logged with an assigned severity and updated to their final state through the end-user community defect review process.

1 Unadjudicated bugs are logged in the system with an initial severity. Upon adjudication through the end-user community defect review process, severity will be updated to its final state.

2 Prime mission vendor escape rate is tracked based on detection source (vendor to DT&E), not by release cycle. Scope for escape rate is severity 1 and 2 issues.

3 Sev 1+2 is the count of severity 1 and severity 2 issues detected at DT&E, which is the scope of the escape rate calculation (see footnote 2).

4 EIDS defect data reflects partial coverage, as EIDS does not use DT&E systems for over-the-shoulder testing.
