2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Stryker SLX628 Surgical Light is the only light system compatible with OKC VAMC s current operating room configuration. The surgical lights will replace lights that are not currently functioning in operating room 1 & 2. The Stryker SLX Surgical Light features 104 LEDs that produce up to 650 beams of light that overlap, creating a consistent and uniform column of light that reduces shadows on the surgical field. It also has a high illumination level of up to 160,000 lux and offers color temperature settings ranging from 3600K to 5000K. The light includes a built-in laparoscopic ambient light feature and a 360-degree light head rotation. Its lightweight and durable aluminum construction, along with the surgical glass lens, prevent burns and stains and minimize downtime and failures. When compared to other brands, the Stryker SLX is the only surgical light that provides 104 LEDs, a 29.6-inch light head diameter, and glass lens construction. Equipment is currently in place in the operating room to support the Stryker lights. Purchasing a different brand would incur additional costs as the operating room would need to be reconfigured. The cost to reconfigure the operating room would exceed the cost of purchasing the Stryker lights as additional equipment would also need to be purchased. Furthermore, the Stryker SLX628 Surgical Light is proprietary and available only through Stryker, the Original Equipment Manufacturer (OEM), or one of their authorized SDVOSB distributors.
4. Description of market research conducted and results or statement why it was not conducted:
Market Research included a search of Vendor Information Pages (VIP), Small Business Administration s Dynamic Small Business Search (DSBS), GSA, and SAM.gov to identify potential vendors. A search of FPDS displayed purchases for Stryker surgical lights were awarded to Stryker Sales LLC (Large), Beacon Point Associates LLC (SDVOSB), or TrillaMed LLC (SDVOSB). Furthermore, VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

(2) authorized SDVOSB distributors, Beacon Point Associates LLC and TrillaMed LLC. Both SDVOSBs can meet the four prongs of the nonmanufacturer rule due to a class waiver for NAICS 335122. VA Rule of Two is met for a SDVOSB set-aside IAW FAR Part 13.
