About this file

This document is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania, specifically related to Executive Order 2021-06. The form requires contractors and grantees to certify compliance with various Pennsylvania state labor and workforce safety laws, including acts covering unemployment compensation, workers' compensation, minimum wage, prevailing wage, employment of minors, equal pay, and several other labor-related regulations.

The certification mandates that contractors affirm their adherence to the listed labor laws and understand potential penalties for false certification, which can include contract termination and a three-year prohibition from bidding on Commonwealth contracts. Contractors must sign the document, print their name, title, and company name, and acknowledge that any changes in compliance status must be immediately reported to the purchasing agency. The form carries legal weight, with potential consequences under 18 Pa. C.S. § 4904 for unsworn falsification to authorities.
