About this file
This document is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania, specifically related to Executive Order 2021-06 on Worker Protection and Investment. The form requires contractors and grantees to certify compliance with various Pennsylvania state labor and workforce safety laws, including 13 specific statutes covering areas such as workplace misclassification, employment of minors, minimum wage, prevailing wage, equal pay, and other employment-related regulations.
The certification form establishes legal consequences for non-compliance, including potential contract termination and a three-year prohibition on bidding for future contracts. Contractors must affirm their adherence to these labor laws and report any changes in compliance status to the purchasing agency. The document includes a signature section where an authorized official must print their name, title, and the contractor/grantee name, and sign to verify full compliance with the specified labor and workplace safety regulations. False certification can result in penalties under Pennsylvania's legal statutes for unsworn falsification to authorities.
