About this file
This document is a Worker Protection and Investment Certification Form related to a Commonwealth of Pennsylvania state and local contract opportunity. Pursuant to Executive Order 2021-06, the Commonwealth requires contractors and grantees to certify compliance with Pennsylvania's labor and workplace safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, and various other state labor and workforce safety statutes. The contractor must provide an authorized signature certifying compliance and understanding that providing false information can result in contract termination and three-year ineligibility to bid on Commonwealth contracts. The document outlines the specific state laws that contractors must comply with, as well as the penalties for submitting false certifications.
The form does not provide details on a specific contract opportunity, but rather establishes the requirements for contractors seeking to do business with the Commonwealth of Pennsylvania. There are no details included regarding product or service requirements, response dates, award information, contract terms, or pricing. The focus of the document is solely on the certification of compliance with Pennsylvania labor and workplace safety laws.
