About this file
This is a Single Source Justification for Simplified Acquisition Procedures (SAP) under the Simplified Acquisition Threshold (SAT) from the Detroit VA Medical Center. The justification supports the procurement of eleven (11) Medfusion 4000 Syringe Pumps - seven replacements and four new units - for the Anesthesia department.
The document justifies sole source procurement of the Medfusion 4000 model based on several factors: its proven accuracy (+/-2% delivery accuracy), specialized capabilities for administering medications like Flolan/Epoprostenol, existing staff familiarity and training, wireless EMR integration, and consistency with facility standards and protocols. Market research found 668 SD/VOSB vendors under NAICS 339112, but none specifically for syringe pumps. While the VA Rule of Two could not be met, the SBA Rule of Two was achievable through Smiths Medical's SDVOSB vendors. The procurement was approved under FAR 13.106-1(b) by Contracting Officer Percy Johnson at the Detroit VA Medical Center.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT
Last Updated: 02/01/22 Page 1 of 2 Revision: 00 Acquisition ID#: 36C250-25-AP-1025
DEPARTMENT OF VETERANS AFFAIRS
Justification for Brand Name Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 36C250-25-AP-1025
1. Contracting Activity: Department of Veterans Affairs, VISN 10, Detroit VA Medical Center, located at 4646 John R St, Detroit MI 48201
2237: 553-25-1-063-0016
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
Anesthesia is requesting the replacement of (7) and the addition of (4) new Medfusion Syringe Pumps model 4000. These pumps have the capability to administer fluids in small volumes. The delivery accuracy is nominal +/-2% with an infusion rate range of 0.01 mL/hr to 1130 mL/hr. This allows the Anesthesia provider to administer fluids such as medications, blood and blood products, lipids, internal solutions as well as other therapeutic fluids to patients in more controlled intervals.
The Medfusion is designed to provide accurate controlled delivery of crucial medications in anesthesia such as vasoactive drugs, opioids, and muscle relaxants in precise doses. It provides safety features such as customizable drug libraries, dose error reduction systems, and alerts for potential infusion errors. The wireless connectivity integrates with hospital systems including EMR and pharmacy ensuring accurate recording therefore reducing the risk of errors.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Medfusion 4000 is the pump that aligns with facilities needs, standards and policies. The anesthsia team has adopted policies and procedures for medication administration based on the use of this Medfusion pump. The pump meets the needs for protocols and drug library configurated needed by the anesthesia team to ensure consistenty in use and reduction in human error. The pump has a proven track record within the anesthsia deparment for reliabliey and accuracy, particulaty in complex anesthesia cases. Deviation may compormise consistency in care.
Also, it is the only syring pump that can administer the drug Flolan/Epoprostenol. All of our anesthesia providers are already familiar and trained on the use of this particular machine and with the upgrade would need minimal to no additional training, using the same model will reduce the potential for user errors that can arise from using unfarmiliar or less intuitive devices. This reduces the need to introduce additional supplies and retraining, which may increase the risk of error. Also this particular model is used at other facilities in the area as well ensuring safe use by trainees.
Going with a different brand/MFR would cause a grave delay in patient care as the staff would need training on the use of such critical equipment.

4. Description of market research conducted and results or statement why it was not conducted:
Searched VetCert using NAICS 339112 and located 668 SD/VOSB vendors. Searched DSBS with NAICS 339112 and keywords Syringe Pump, and there were 0 SD/VOSB vendors found. Requested items were not available on GSA or VANAC. Contracted Smiths Medical (OEM, LB) and was advised they have several SDVOSB vendors for this product. The VA Rule of Two is not attainable for this procurement. The SBA Rule of Two can be met.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
