About this file
This is a Commercially Useful Function (CUF) Certification form issued by the State of California Department of Insurance for certified Disabled Veteran Business Enterprise (DVBE), Small Business (SB), and Micro Business (MB) entities. The form, identified as CDI-FBMD-BMB 008, requires completion by any certified SB, MB, or DVBE that will perform an element of work on a contract. All respondents must demonstrate their capacity to execute a distinct element of work, carry out contractual obligations through actual performance, management, or supervision, perform work that is normal to their business operations, and avoid excessive subcontracting beyond normal industry practices. A negative response to questions one through three or an affirmative response to question four regarding unexpected subcontracting may result in disqualification of the bid as non-responsive. Additionally, respondents must provide a written statement detailing the role, services, and goods their company will provide to satisfy CUF requirements.
The form mandates authorization by the certified business owner or authorized corporate representative, who must certify under penalty of perjury that all information provided is truthful and accurate. The document references compliance with Government Code section 14837(d)(4) for Small Businesses and Military and Veterans Code section 999(b)(5)(B) for DVBEs. At the State's discretion, firms may be required to submit additional clarifying information prior to contract award. The form includes fields for OSDS reference numbers and certification expiration dates for currently certified firms.
