About this file
This is a Commercially Useful Function (CUF) Certification form issued by the California Department of Insurance (CDI) for use by certified Disabled Veteran Business Enterprises (DVBEs), Small Businesses (SBs), and Micro Businesses (MBs) performing work on state contracts. The form applies to the Outreach and Advertising Services Request for Proposal (RFP # 26CROB-001S), which covers services for the California Low-Cost Auto Insurance Program (CLCA), Patient and Provider Protection Act (PPPA), and Life and Annuity Consumer Protection (LACPP) Programs. The CDI intends to award one contract for a three-year term with an optional one-year extension. The written question submittal deadline is February 11, 2026, at 5:00 PM, with final proposal submissions due March 4, 2026, at 4:00 PM.
Any certified SB, MB, or DVBE prime contractor, subcontractor, or supplier must complete this certification form to demonstrate compliance with CUF requirements under Government Code section 14837(d)(4) and Military and Veterans Code section 999(b)(5)(B). The form requires respondents to affirm that their business will be responsible for executing a distinct element of work, will carry out contract obligations through actual performance, management, or supervision, and will perform work normal to their business functions. Responses of "No" to questions one through three or "Yes" to question four regarding excessive subcontracting may result in the quote being deemed non-responsive and disqualified. Respondents must provide a written statement detailing their role, services, and goods to satisfy CUF requirements and must be authorized by the certified business owner or authorized corporate representative under penalty of perjury.
