About this file

This document is a Bid Submission Checklist for a contract opportunity issued by the Maryland Department of Health (MDH) to provide part-time Licensed Speech Therapy services to the intellectually disabled individuals at the Potomac Center and Secure Evaluation & Therapeutic Treatment (SETT) facilities in Hagerstown, Maryland. The contract requires a Speech Therapist with at least one year of experience working with the intellectually disabled and/or dually-diagnosed, along with specific licensing requirements from the Maryland State Board of Examiners and the American Speech Language and Hearing Association (ASHA). The contract term is one year (52 weeks) starting on 7/13/2022, with services needed for an average of nine hours per week at the two facilities.

The bid submission requirements include providing a completed Financial Form (Exhibit A), Bid/Proposal Affidavit (Attachment C), the Speech Therapist's resume, one reference, one letter of recommendation, copies of the required licenses, and proof of the agency's registration and good standing with the Maryland Department of Assessments and Taxation, if the bidder is an agency or group. Bids may be submitted electronically via email or through the eMMA system, and failure to include all mandatory submissions will deem the bid non-responsive.
