About this file

This is a Responder Information and Certification Sheet for Request for Qualifications (RFQ) Bid S-58100-00017037, issued by the State of Oregon Department of Administrative Services (DAS) Procurement Services for an Early Literacy Provider List. The document serves as an attachment (Attachment D) to the RFQ and requires potential responders to provide organizational information including legal name, address, state of incorporation, entity type, contact information, and Oregon Business Registry Number. Responders must also document their years of experience providing facilitation services in education for school districts, Education Service Districts, or senior-level administrative positions in education. The RFQ requires submission of a firm offer valid for 180 days following the opening date, with responders agreeing to be bound by contract terms and conditions outlined in Attachment A and any amendments issued by the Agency.

The certification sheet establishes mandatory compliance requirements for all responders, including adherence to Oregon tax laws, non-discrimination policies in employment and subcontracting practices, wage payment equity standards under ORS 652.220, and broadband service provision compliance if applicable. Responders must certify they are not listed on the Office of Foreign Assets Control's Specially Designated Nationals list or System for Award Management exclusion lists, and must disclose any actual or potential conflicts of interest. For contracts with an anticipated price of $150,000 or more, responders must certify implementation of written policies and practices preventing sexual harassment, sexual assault, and discrimination against protected class members as required by ORS 279A.112. All certifications are made under penalty of the Oregon False Claims Act, with false or fraudulent statements potentially resulting in liability and penalties.
