About this file

Summary

This document is a Seller Quote Checklist for Public Works Bids issued by the City of Philadelphia Procurement Department for Bid 2746, which concerns the relay of water mains on Lott Avenue in Philadelphia, Pennsylvania. The checklist serves as a comprehensive submission guide for vendors approved to bid on this public works project, which has an estimated value exceeding $5,000,000. The document outlines a multi-step submission process across several tabs within the PHLContracts electronic bidding platform, requiring bidders to complete the General, Items, Questions, Terms & Conditions, Attachments, and Summary tabs. All listed documents and information are mandatory requirements, and failure to submit any required item will result in disqualification from the bidding process.

The submission requirements include several critical components: completion of the Local Business Entity Determination Form if applicable; entry of itemized costs with submission of a mandatory Pricing Page; payment of a $600 bid processing fee submitted electronically through the City's official payment portal before the bid opening date; execution of a Consent & Authorization form; submission of a bid bond and Power of Attorney (with the original bid bond mailed separately to the Procurement Department); completion of additional worksheets such as the Unit Prices Worksheet and Performance of Work by Contractor form; and execution of a Project Labor Agreement if applicable. Bidders must acknowledge all amendments, confirm compliance with bid requirements, accept all terms and conditions without exception, and review all attachments listed on the Terms & Conditions tab. No exceptions are permitted on public works contracts, and all submissions must be completed through the online platform with required documentation uploaded and, where specified, mailed in original form prior to the bid opening date.
