About this file
This is an Indian Affairs Indian Economic Enterprise Representation Form used for federal contract solicitations under the Buy Indian Act (25 U.S.C. 47). The form serves as a self-certification document that an offeror meets the definition of an "Indian Economic Enterprise (IEE)" as defined in the Department of the Interior Acquisition Regulation (DIAR) Part 1480. By submitting this form, the offeror certifies that it satisfies the IEE requirements at three critical points: when the offer is submitted in response to a written solicitation, at the time of contract award, and throughout the full term of the contract. The form requires offerors to provide their name of federally recognized tribal entity, legal business name, unique entity ID (UEI), the name of the 51% (or greater) owner of the IEE firm, along with a certifying signature and printed name.
Contracting Officers retain the authority to request additional information or documentation of IEE eligibility at any point during the acquisition life-cycle, including during pre-award and proposal evaluation phases. The form emphasizes that false or misleading information submitted in connection with a Buy Indian Act set-aside award violates federal law under 18 U.S.C. 1001, while false claims submitted during contract performance are subject to civil and criminal penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287. The form itself is a template with blank fields for solicitation number, project title, and date of form submission.
