BLK 4:
a) Contractor format acceptable.
b) The most current version of the Data Item Description is to be used.
c) Include all Government Furnished Equipment and Facilities required and the date and duration they will be required. Include a cost chart depicting by month the estimated cost, including subcontractor costs, for each major task.
d) Draft and Final Program Plans shall be marked with the appropriate distribution statement and any restrictive legends that apply. If restrictive legends are not evident, the data will be treated as unrestricted.
e) An electric copy of the Program Plan shall be delivered to each recipient on one properly labeled CD-ROM or via the DoD SAFE Site (https://safe.apps.mil/).
f) The most commonly used version of MS Word shall be used.
BLK 9: All documentation shall be marked IAW DoDI 5230.24 with the following distribution statement: “DISTRIBUTION STATEMENT C. Distribution authorized to US Government agencies and their contractors; (reason), (date of determination). Other requests for this document shall be referred to the Joint Directed Energy Transition Office (DE JTO), 801 University Blvd, Ste 209, Albuquerque, NM 87106.” Contractor shall suggest reasons as appropriate for technology development; more than one reason can be listed: Critical Technology; Export Controlled, Administrative or Operational User. Acceptance of suggested reasons for distribution statement are at the Government’s sole discretion.
EXPORT CONTROL WARNING: All technical data must be marked as follows if appropriate: “WARNING – This document contains technical data whose export is restricted by the Arms Export Control Act (Title 22, U.S.C., Sec 2751 et seq.) or the Export Administration Act of 1979, as amended, Title 50, U.S.C. BLKS 10-13: The contractor shall deliver a draft Program Plan within 30 calendar days after contract award. The DE JTO will review and return with comments no later than 15 days after receipt of the draft Program Plan. Upon receipt of the comments, the contractor shall incorporate all required changes and deliver the final version of the Program Plan within 15 days after receipt of DE JTO comments. The plan shall be updated, as required, from the date of initial acceptance through technical completion.
BLK 14: The contractor shall distribute in accordance with BLK 14. When the addressee’s copy is identified by “Ltr,” send ONLY a copy of the transmittal letter to that addressee. The transmittal letter shall not include any restricted data and should therefore be sent via email.

THE CONTRACTOR SHALL NOT SUBMIT THE FINAL REPORT TO DEFENSE TECHNICAL INFORMATION CENTER (DTIC). Only DE JTO will submit.
BLK 4:
a) The scientific and Technical Report shall be prepared IAW the most current AFRL ANSI/NISO Standard Z39.18-2005, or other most current report preparation guidance.
b) The contractor shall complete and submit an SF298 Report Documentation Page, with the final report. The SF 298 is available at http://dtic.mil/pdf/formsnguides/sf298.pdf. The contractor shall complete the following blocks of the SF298: Blocks 2-5a, 6, 7, and 13 (if applicable), 14 – 16c. Complete the following boxes as shown:

c) An electronic copy shall be delivered for both the draft and final version of the report to each recipient on one properly labeled CD-ROM or via the DoD SAFE Site (https://safe.apps.mil/). The electronic copies shall be an editable .pdf file and the most current version of Microsoft Word.

Incomplete final reports will be immediately returned to the contractor. The DE JTO and technical monitor(s) will review, edit and return the draft no later than 30 calendar days after receipt of the draft Final Report. Upon receipt of the reviewed and edited draft, the contractor shall incorporate all required changes and deliver the final version within 30 calendar days after receipt of government comments.

Blk. 4: GFP/CAP Inventory Reports shall be in accordance with DFARS PGI 245.402-71. If government furnished property or equipment is provided, the list shall be in accordance with DFARS PGI 245.103-72. Abbreviations are not allowed. Deliver an electronic copy as a Microsoft Excel document. A CD-ROM is acceptable media for submittal. Unclassified documents may be submitted by encrypted or password protected e-mail, or other secure file transfer method. E-mail encryption based on PGP platform is not acceptable.
Blk. 8: Ninety (90) days prior to the contract expiration date, the contractor shall deliver a “Draft” GFP/CAP Inventory Report in the format required by DFARS PGI 245.103-72, suitable for contract close-out purposes. Include the use/disposition recommendations for review and approval by the Government Program Manager, Contracting Officer, and/or the DCMA Property Administrator. The Government Program Manager will review and if necessary, return the list with comments within 30 calendar days.
Blk. 11 & 12: The contractor shall deliver an initial Baseline GFP/CAP Inventory Report within 30 days of the first inclusion of GFE/CAP in the contract. After the baseline is established, the property inventory shall be submitted semi-annually. Additional distributions may be requested by the Government Program Manager or the Contracting Officer.
Blk. 13: Thirty (30) days prior to the contract expiration date, the contractor shall deliver the “Final” GFP/CAP Inventory Report, representing all property delivered in accordance with the contract.
