About this file

This document is a Banking Questionnaire requesting information about a contractor's bank account from the referenced financial institution. The purpose is to ensure the contractor's financial responsibility prior to awarding a federal contract. The questionnaire asks the bank representative to provide the date the contractor's account was opened and to respond to three questions regarding the contractor's account status, credit standing, and average monthly balance. The completed questionnaire must be emailed directly to the Contracting Officer at kxsua@bop.gov by November 22, 2024, as it is required for the bid opening on that date.

The related federal contract opportunity is for the Emergency Repair Generator and High Voltage Line project at the Federal Correctional Institution (FCI) in Phoenix, Arizona. The project consists of the removal, repair, and re-installation of an existing 2000 KW generator, as well as the provision of trenching, conduit, and high voltage line installation. The NAICS code for this requirement is 238210, with a small business size standard of $19 million. The estimated magnitude of the project is between $1,000,000 and $5,000,000. This is a 100% small business set-aside, and all bidders must be registered in SAM.gov with the appropriate NAICS code.
