RE: Hazardous Materials Assessment Report
5163 Old Dixie Hwy
Forest Park, Clayton County GA 30297
CERM Project No. 2023-1511A-001A
Dear Mr. Messinger, Corporate Environmental Risk Management, LLC (CERM) was retained by STV, Inc. to conduct a Pre-
Demolition Hazardous Materials (HazMat) Assessment at 5163 Old Dixie Hwy, Forest Park, Clayton
County, GA 30297. At the time of the assessment, there were two tenant spaces in the building. The
SCP Distributors (Pool Supply Company) section of the building was an unoccupied office space and a vacant warehouse. The Tsunami Volleyball area was currently in use and occupied by the tenant.
Mr. Lorenzo Gates, Darryl Edler, and Andre Cumberbatch of CERM, conducted the on-site sampling and evaluations of the Hazardous Materials Assessment on April 3, 2023 and April 12-13, 2023. The
HazMat Assessment of this facility was coordinated with the HazMat Assessment of the south adjacent
5265 Old Dixie Hwy, an occupied secured-access facility requiring an escort. The assessment reports are provided under separate cover. No as-built drawings were available for use by CERM in performance of this assessment.
SCOPE OF SERVICES
The Scope of Services for the above referenced property included a Pre-Demolition Hazardous
Materials Assessment. The HazMat assessment included the following tasks:
1) Asbestos-Containing Materials (ACM) Sampling;
2) Lead-Based Paint (LBP) Testing; and
3) Universal Waste Inventory for PCB-containing equipment (i.e., light ballasts, electrical transformers, hydraulic equipment), mercury-containing equipment, fluorescent light bulbs, and other chemical storage containers.
All work was performed in accordance with applicable state and federal guidelines and industry standards.

SAMPLING METHODOLOGY
SUSPECT ASBESTOS-CONTAINING MATERIALS
All accessible areas of the building were observed for suspect asbestos-containing materials (ACM).
Small pieces of each observed suspect ACM were collected using a metal chisel, and/or other means, including a hammer where necessary. Each sample was placed in an individual plastic container and given a unique sample identification number. The sample number, material location, and material description were recorded on a field survey log. In accordance with Environmental Protection Agency
(EPA) guidelines, multiple samples were collected of each homogeneous (same color, texture, and/or application date) area (material). As a general rule, when one of multiple samples of a homogeneous material yields a result >1%, the material is considered an ACM. The samples were transported to
Analytical Environmental Services, Inc. (AES) for analysis of total asbestos content (% by volume).
SUSPECT LEAD-BASED PAINT
All accessible areas of the building were observed for suspect lead-based paints (LBP). Suspect painted surfaces were tested for lead content by hand-held instrument or by paint film collection for laboratory analysis. CERM utilized a handheld Niton XL2 GOLDD X-Ray Fluorescence (XRF) Analyzer by
Thermo Scientific to collect direct readings of painted surfaces throughout the subject building. If a measurement of a painted surface by XRF yields a reading at or above 1.0 milligrams per square centimeter (mg/cm ) that paint film is considered lead-based paint.
Small pieces of each observed suspect LBP were collected using a utility knife, metal chisel, and/or other means. Each sample was placed in an individual plastic container and given a unique sample identification number. The sample number, material location, and material description were recorded on a field survey log. Representative samples of each suspect LBP were collected for analysis. The samples were transported to Analytical Environmental Services, Inc. (AES) for analysis of lead content
(% by weight).
UNIVERSAL WASTE INVENTORY
The inspection of accessible areas of the building for other hazardous materials such as stored chemicals, PCB light ballasts and mercury-containing equipment consisted of identifying and characterizing known or suspected hazardous materials.
Phone: 678-999-0173 ● Fax: 678-999-0186 cerm.com 3
LABORATORY RESULTS & FINDINGS
The suspect ACM samples collected consisted of drywall/joint compound material, ceiling tile, flooring materials, thermal system (pipe) insulation (TSI), and roofing materials. The samples were transported to AES under chain of custody for analysis. The samples were analyzed by Polarized Light
Microscopy (PLM) coupled with dispersion staining techniques in accordance with the EPA “Method for the Determination of Asbestos in Bulk Building Materials, EPA/600/R-93/116, July 1993”.
EPA/NESHAP regulations define an asbestos-containing material (ACM) as a material containing greater than one percent (>1%) asbestos in a bulk sample. CERM collected fifty-eight (58) samples of suspect asbestos-containing materials. The results are summarized in Table 1: Suspect Asbestos-
Containing Materials (ACM) Results. The laboratory analytical report and completed chain of custody are attached.

Laboratory analysis of bulk samples collected at the 5163 Old Dixie Hwy warehouse building did not detect the presence of asbestos in any of the samples collected. However, mirrors on the walls of the Women's and Men's Restrooms in the Tsunami space are assumed to be adhered to the wall by asbestos-containing glue dots. Investigating for the presence of glue dots was not possible without removing the mirrors and incurring risk of damaging them in this occupied tenant space. The laboratory analytical report and completed Chains of Custody are attached. (See the attached
Laboratory Analytical Results Report for a detailed listing of all the samples that were examined at

A lead concentration greater than or equal to 0.5% by weight was not detected in any of the suspected lead-based paint samples collected at 5163 Old Dixie Hwy.
Renovation or other activities conducted by employees (of any entity) that impact the materials or otherwise expose employees to lead are governed by OSHA (29 CFR 1926.62). EPA also has testing requirements that apply to the waste stream of building materials that contain lead as characterization for disposal (i.e., landfilling/incineration). EPA regulations and HUD guidelines will apply if the facility is sold or renovated for reuse, particularly as a “child-occupied facility”.

RECOMMENDATIONS
The presence of (assumed) asbestos-containing glue dots in the Tsunami space Women's and Men's
Restrooms should be confirmed prior to demolition of the building. If determined to be asbestos-containing material, the mirrors and adhesive dots should be removed and disposed of by a Georgia-licensed asbestos abatement contractor. CERM recommends that any portion of the building material waste-stream that includes lead-based paint should be characterized by toxicity characteristic leaching procedure (TCLP) laboratory analysis in order to determine whether it must be disposed as hazardous.
CERM recommends that fluorescent light bulbs and mercury-containing thermostats, where applicable, be submitted to a licensed recycling facility. These items should be contained in sealed packages for transport. EPA recommends that these items be handled by trained professionals. CERM recommends that all suspect PCB-containing light ballasts be removed, contained in sealed drums and shipped to a licensed incineration facility for disposal. For occupied facilities, federal law requires that any suspect
PCB-containing light ballasts that are found to be leaking be immediately removed and disposed of.
Department of Transportation (DOT) requirements may also apply.

All analyses performed in accordance with EPA “Interim Method for the Determination of Asbestos in Bulk Insulation Samples” (EPA 600/M4-82-020), 1982 as found in
40 CFR, Part 763, Appendix E to Subpart E and “Method for the Determination of Asbestos in Bulk Building Materials” (EPA/600/R-93/116), 1993.
These test results apply only to those samples actually tested, as submitted by the client. All percentages are reported by visually estimated volume.
PLM is not consistently reliable in detecting small concentrations of asbestos in floor tiles and similar nonfriable materials, quantitative TEM is currently the only method that can be used to determine conclusive asbestos content.
