Performance Work Statement (PWS)

Repair failed transformer for Building 301 Sub 2

1. Scope: The work covered by this Performance Work Statement (PWS) consists of providing transportation, equipment, materials, and performing all labor required to repair/rewind failed 250kVA Dry type 13.8kV – 120/240V Single phase transformer.

2. Location of Work: The pickup and return location is 106 Peacekeeper Dr, Facility 301, Offutt AFB, Nebraska.

3. Major Items of Work:

Transport transformer:

· Pickup from and return to 106 Peacekeeper Dr, Offutt AFB, NE 68113.

Repair transformer:

• A full inspection of the unit will be performed once it arrives at the factory

• Remove Core & Coil from Enclosure if required. (See Notes section for details)

• Disassembly of core and coil assembly.

• Thoroughly clean Core and all structural parts to remove any dust or contaminants & baked dry.

• Replacement / manufacturing of all original Cast coils (Phase A & B) of transformer with New VPI Coils.

• Replace all required insulators that may have damage and/or as required for new coils construction.

• Replace existing phase barriers along with cables and jumper leads.

• Clean all Bus Work

• Reassembly of complete core and coil assembly.

• Dip and Bake Core & Coil Assembly in Isonel for complete Environmental Protection.

· Preform IEEE test and provide customer with certified test report

• Prep unit for return shipment

4. Codes: All work and personnel shall conform to the latest applicable NFPA, OSHA, Air Force Instructions (AFI), United Facilities Criteria (UFC) 3-535-01, and other Industry Standards.

5. Occupancy: During maintenance the Government will occupy the existing facilities. The contractor shall not restrict nearby pedestrian or vehicular traffic to this facility and shall coordinate construction with the Contracting Officer’s Representative to minimize conflicts and to facilitate usage. The contractor shall perform the work so as not to interfere with ongoing operations.

6. Coordination: The contractor shall coordinate and properly relate the work to be done to the site and to the work of all the trades. The contractor shall visit the premises and thoroughly familiarize him or herself with all the details of the work to be done and the working conditions. The contractor shall verify all of the dimensions in the field and advise the Contracting Office of any discrepancy before performing any work. If any conflicts arise during work necessitating departure from the Performance Work Statement (PWS), details of the departure and the reasons for the departure shall be submitted to the Contracting Office for written approval.

7. Submittals: The contractor shall submit two copies of inspection certifications and any applicable letters of findings to the Contracting Office.

8. Repair Work: The work shall be carefully laid out in advance. Where cutting, channeling, chasing, or drilling is necessary, it shall be done carefully. Any damage to buildings, piping, or equipment, shall be repaired by workers skilled in that particular trade, at no additional cost to the Government.

9. Clean Up: The contractor shall assure that the work area is cleaned up at the end of each day. Debris and trash shall be removed and properly disposed of by the contractor outside the confines of Offutt AFB.

10. Work Schedule: The working hours for this contract shall be between 0730 and 1630 hours daily, excluding Saturdays, Sundays, and Federal Holidays.

11. Performance Period: 30 Days

12. Use of Utilities: All reasonable quantities of utilities will be made available to the contractor without charge. Any temporary connections of lines that may be required will be installed, maintained and removed by the contractor at his own expense and in a manner satisfactory to the Contracting Officer. Removal of such connections or lines shall be accomplished prior to final acceptance of the contract.

13. Guarantee: The equipment furnished under this contract shall be guaranteed against defective materials, design, and workmanship for the full warranty period, which is standard with the manufacturer and/or supplier, but shall be no less than one year from the date of acceptance either for beneficial use or final acceptance (whichever is earlier). Upon receipt of notice, from the Government, of failure of any part of the guaranteed equipment, during the guarantee period, the affected part or parts shall be replaced promptly with new parts at the expense of, and by, the contractor.

14. Payment: All costs in connection with this contract shall be included in the lump sum price for all work to be performed under the contract.

15. Government Furnished Equipment: None

16. Security and Safety Requirements: The contractor shall comply with all requirements and procedures IAW FAR 5353.242-9000, Contractor Access to Air Force Installations.

Personal Security Requirements

• Contractor personnel shall be in possession of Government issued photo identification at all times while on Offutt AFB, and coordinate with Contracting Officer Representative (COR) through the Contracting Office for entry onto the Security Force’s Visitors List.

• Except where otherwise posted, the speed limit on Offutt AFB is 25 MPH. Seatbelts shall be worn at all times by driver and all passengers. Cellphone use is prohibited while driving anywhere on Offutt AFB, except while using hands-free device. All vehicles and personnel are subject to search at any time while on Offutt AFB.

Restricted Area Badges

• If necessary, the COR shall coordinate with Using Agency Facility Manager (UAFM) to obtain a United States Air Force (USAF) Restricted Area Badge for all personnel who require unescorted entry into a restricted area(s) for specific work, for the duration of the contract. It is the responsibility of the UAFM to assist the contractor and his/her employees in initiating a DD Form 254, DOD Contract Security Classification Specification, through the Unit Security Manager (USM). If for any reason the UAFM cannot assist the contractor in processing paperwork for restricted area badges, it is their responsibility to provide escorts. It is the contractor’s responsibility to fully document all cases where entry to restricted areas was not granted. All contractor personnel shall have proper identification prior to entry into restricted areas. It is the responsibility of each UAFM to brief all contractor personnel working in their area on procedures and policies of their particular restricted area.

17. Final Inspection: Contractor will remove material and construction equipment from the site. Contractor will provide submittals and turn in appropriate badges.
