About this file
This is an Office Automation Form A for a state and local contract opportunity in Pennsylvania involving the City of Philadelphia and the procurement of office automation equipment, specifically a Lexmark MS823dn multifunction device. The form establishes requirements for equipment installation, configuration, warranty coverage, and maintenance services. The vendor must provide product delivery with configuration completed at the City's location and submit product literature and manufacturer documentation upon request. All deliveries must be inside deliveries to the facility rather than drop shipments to loading docks.
Warranty coverage requirements include a minimum five-year warranty for servers and IT appliances, four-year coverage for personal computers and laptops, and three-year coverage for tablets, with all warranties extending beyond a 30-120 day acceptance period. The vendor must provide on-site maintenance with next-day response capability within 24 hours, including all parts and labor. The form indicates that instruction and training by the vendor are requested, and the vendor may offer either refurbished or new equipment. Payment for maintenance options is due after equipment delivery and City acceptance. The vendor must cover all costs associated with installation, configuration, and delivery unless otherwise directed by the City.
