About this file
This is a Statement of Need for the supply, delivery, and installation of two 8-station clinical audiometric testing booths at Blanchfield Army Community Hospital, Fort Campbell, Kentucky. The project is a complete turnkey operation that includes decommissioning and removal of existing audio booth assets. Booth 1 (Room BG02) must fit within maximum dimensions of 122" width, 94.5" height, and 128.5" depth, and must be ADA-compliant with a wheelchair access ramp and low-profile isolated floor system built on top of the concrete slab. Booth 2 (Room BA04) must fit within maximum dimensions of 113.5" width, 100" height, and 166.5" depth, and must feature a standard vibration isolator as it will be recessed into the concrete slab. Both booths require internal lighting and configuration for a dedicated 120-volt, 15/20 AMP, 60 Hz electrical supply circuit.
The contractor must provide all labor, tools, and transport to disassemble, safely remove, and dispose of or recycle two existing IAC Acoustics Model 40M-8 audio booths (Serial Number 10-2158 in Room BG02A and Serial Number 1001 in Room BA04). Upon disassembly, the decommissioned assets become contractor property, and any scrap or resale value may be factored into pricing. The document includes facility layouts showing both rooms and a photograph depicting the existing bulkhead and wheelchair ramp configuration in Room BG02.
