Section SF 1449 - CONTINUATION SHEET
SOLICITATION NARRATIVE
Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Products and Commercial Services (NOV 2021)
1. The National Guard Bureau (NGB) Directorate of Acquisitions, Operational Contracting Division (AQ-C) hereby issues Request for Proposal (RFP) / solicitation W9133L22R1004 in support of the Air National Guard (ANG) Chief Surgeon’s Office (CSG) for Centralized Credentialing and Privileging (C&P) Services.
2. This requirement is being competed in accordance with Federal Acquisition Regulation (FAR) Parts 12, Acquisition of Commercial Items, and FAR Part 15, Contracting by Negotiation, as a full and open, unrestricted competition, with no small business set-aside. The Government intends to make an award without conducting discussions; however, it reserves the right to conduct discussions and request other than certified cost or pricing data as needed/required.
3. The Government intends to issue a single firm-fixed price (FFP) award for a one-year (12-month) base period, with four one-year (12-month) options in accordance with FAR 52.217-9, Option to Extend the Term of the Contract, for a total of five years (60 months). In addition, the resulting award will include an option for up to a six-month extension in accordance with FAR 52.217-8, Option to Extend Services.
4. Award shall be made based on the best overall (i.e., best value) proposal that is determined to be the most beneficial to the Government. All contract line item numbers (CLINs), as provided in the Pricing Workbook (Attachment 0002), will be evaluated as part of the award decision for this requirement. The Government reserves the right to make no award, depending on the quality of proposals received, prices submitted, and the availability of funds. An award under this solicitation in no way requires the Government to exercise any options.
5. The Contractor is required to meet all contract performance requirements in accordance with the Performance
Work Statement (PWS), Attachment 0001. If the requirements take more staffing than proposed (without a change by the Government), the Contractor is required to provide additional staffing at no increase in price, until all contract performance requirements are satisfactorily met.
6. All efforts shall be performed in accordance with Attachment 0001. The specific portions of the Contractor’s technical proposal that exceed any of the Government’s requirements provided in the Schedule or PWS (including, but not limited to dates, performance, values, and/or any other measurable criteria) may be incorporated into the contract and supersede the Government’s threshold requirements that were exceeded and/or the Government’s objective requirements that were met and/or exceeded. Moreover, the Government reserves the right to incorporate any other aspect of the successful offeror’s proposal into the contract at its sole discretion. Acceptance of the successful Offeror’s proposal does not waive any of the requirements set forth in the resulting contract and corresponding attachments.
7. All documents must be handled in accordance with the markings and restrictions contained on each document. If an Offeror does not receive an award from the RFP, the Offeror agrees to immediately destroy all documents associated with this solicitation. In addition, the Offeror must notify any and all subcontractors and/or affiliates provided this information for the purposes of proposal development to do the same.
8. Proposals shall be valid for 120 calendar days after the solicitation closing date.
9. All computer files shall be virus checked prior to submission. Offerors may be held financially liable for damages to Government computer systems by any virus introduced during review of the submitted documents.
10. Only one proposal per company will be accepted. The Offeror, through its parent, affiliates, subsidiaries, business units, etc. is permitted to submit one proposal in total in response to this solicitation. Entities with parents, affiliates, subsidiaries, business units, etc. must decide which entity will submit a proposal in response to the solicitation.
Definitions:
“Affiliates” are business concerns that are affiliates of each other if, directly or indirectly, either one controls or has the power to control the other, and another concern controls or has the power to control both.
“Company” includes affiliates and business units as defined in FAR 2.101; this also includes Joint Ventures and business concerns in a mentor-protégé relationship pursuant to the SBA Mentor-Protégé Program or the Department of Defense Pilot Mentor-Protégé Program.
“Division” is a separate business unit of a company representing a specific business function.
“Subsidiary” means an entity which more than 50 percent of the entity is owned directly by a parent corporation; or through another subsidiary or a parent corporation.
11. A Joint Venture and individual members in the Joint Venture are permitted to submit one proposal in total in response to this solicitation. Members of the Joint Venture must decide which – the Joint Venture or the individual members – will submit a proposal in response to the solicitation. Please be advised that if the Joint Venture does not submit a proposal, then any of the individual members may submit their own proposal.
12. Offerors are advised to continuously monitor System for Award Management found at SAM.gov for new information. The Offerors are deemed to have knowledge of all documents that are posted to SAM.gov.

c. Content Requirement.
This solicitation does not commit the Government to pay any cost for preparation and submission of a proposal.
In addition, the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with this acquisition.
Offerors shall carefully follow the instructions contained herein to assure timely and equitable evaluation, and to avoid exclusion from further consideration. It is an offeror’s responsibility to submit a well-written proposal, with adequately detailed information which demonstrates its clear understanding of, and compliance with, the solicitation requirements.
Proposals shall be complete and clear in all respects without the need for additional explanation or information.
Offerors shall ensure proposals provide a straightforward and concise delineation of capabilities to perform the contract. Organization, clarity, accuracy of information, and relevance are of prime importance. The proposals shall provide sufficient detail and scope to permit the Government to evaluate them in accordance with the evaluation factors as identified in FAR provision 52.212-2, Evaluation – Commercial Products and Commercial Services (Nov 2021).
Offerors shall confine their proposals to essential matters sufficient to define their offer, which will provide an adequate basis for evaluation. Likewise, where applicable, Offerors are cautioned against general, vague, or unsubstantiated statements, which prevent, or render difficult, the Government’s evaluation. Statements such as, “will comply,” or “noted and understood” without supporting discussion to define compliance are not acceptable. Further, where applicable, proposals that fail to provide the required information or approach to meeting the solicitation’s requirements, are not acceptable and may render the Offeror unsuitable for award.
The Government will not assume that an Offeror possesses any capability, understanding, or commitment that is not specifically delineated and supported in its respective proposal. An offeror who was or is an incumbent or subcontractor for an incumbent for any of the previous contracts is still required to demonstrate in its proposal that the offeror meets the requirements stated in this solicitation. An offeror shall not, based upon the offeror’s incumbency, presume the Government’s knowledge regarding its performance, capabilities, skills, or any other aspect which is not specifically provided in its proposal.
All information shall be confined to the appropriate file. The Offeror shall confine submissions to essential matters, sufficient to define the proposal in a concise manner, to permit a complete and accurate evaluation of each proposal. Each file of the proposal shall consist of a Table of Contents and the Narrative discussion.
The Summary Section shall contain a brief abstract of the file. Proprietary information shall be clearly marked. The following shall be included in each volume narrative discussion:

“I, (Name and Title), warrant that: 1) I am an official authorized to bind the entity; and 2) to the best of my knowledge and belief, no actual and potential organizational conflicts of interest have been identified as of (Date).”

(ii) VOLUME II – REQUIREMENTS UNDERSTANDING/APPROACH.
(A) Section 1 – Technical Approach.
(1) For evaluation purposes only, Offerors will not address all of the requirements delineated in the PWS in their Section 1 – Technical Approach. Instead, the Offeror’s proposal shall clearly demonstrate the Offeror’s approach to meet each of the following specific Technical requirements delineated in the PWS:
(a) Credentialing – Initial and Re-credentialing (PWS) Paragraphs 5.3 and 5.4)
(b) Privileging – Initial and Re-privileging (PWS Paragraphs 5.3 and 5.4)
(c) Inter-facility credentials transfer briefs (ICTBs) (PWS Paragraph 5.5)
(d) Credentialing and Privileging Health Risk Management – Adverse Actions (PWS
Paragraph 5.6)
(e) Credentialing and Privileging Quality Assurance and Quality Management (PWS
Paragraph 5.8)
(2) Please be advised, Offerors will be required to perform ALL PWS paragraphs, but for the purposes of this proposal, will only be required to propose the five paragraphs, a through e, above.
(B) Section 2 – Staffing Plan.
(1) Offerors shall provide a staffing table in accordance with Solicitation Attachment 0006, Staffing Table Template, and shall include teaming partners, subcontractors, and locations of staffing. However, it is incumbent upon each contractor to define the labor to be utilized to satisfactorily complete the requirements of the PWS along with the quality of labor to be utilized. The staffing table submitted by the Offeror may be incorporated into the contract at award. Failure to document staffing for each PWS requirement may cause the Offeror’s submission to be evaluated as unacceptable, which would make such submission ineligible for award. The Offerors shall not alter the staffing table to add or delete columns. The Offeror’s staffing table is not subject to the page limitations identified in Table I. However, Offerors shall not include any information in their staffing table template other than information specifically related to Attachment 0006, Staffing Table Template. Including information that is not specifically related to the Offeror’s staffing table may cause the Offeror’s submission to be evaluated as unacceptable, which would make such submission ineligible for award.
(2) Please be advised that the Government does not want Offerors to identify specific individuals.
Rather, the Government seeks to understand the roles individuals will fill. Do not include resumes in any volume; resumes WILL NOT be reviewed.
(3) In addition to providing a staffing table in accordance with Attachment 0006, Staffing Table
Template, the Offeror shall submit a Staffing Approach, which is a separate document and is subject to the page limitations identified in table I. The Staffing Approach must be definitive and provide a clear understanding of how the Offeror intends to meet all requirements throughout the term of the contract, to include teaming arrangements if applicable. At a minimum, the Staffing Approach shall describe the following:
(a) The Offeror shall identify all positions they deem as “Key Billets” within their performance of the contract. Note the Government is not interested in Key Personnel, but rather the billets they fill, as personnel can (and always do) change. Key Billets in this instance are billets that the Offeror considers as significantly influencing successful planning or performance on the contract. DO NOT PROVIDE RESUMES, THEY WILL
NOT BE REVIEWED.
(b) Offerors shall explain their rationale for identifying a position as a “Key Billet.” Offerors are encouraged to tie their rationale back to specific duties and responsibilities.
(c) Offerors shall explain method of incentivizing retention of identified Key Billet; this includes any fringe benefits for SCA-Exempt employees.
(d) For both Key and Non-key Billets, Offerors shall explain strategies to:
i. Recruit and retain a qualified and capable workforce that can accomplish the entire scope of the effort outlined in the PWS; and
ii. Maintain employee’s industry-specific knowledge and skills, continuous learning, and training on new industry technologies.
(e) The Offeror’s response shall also provide any other supplemental description of its Staffing Approach to assist the Government’s evaluation of the Offeror’s Staffing Plan.
For example, this is an opportunity for the Offerors to explain their approach to staffing complexities such as overlapping labor / multiple responsibilities (e.g., “dual-hatted” individuals), part-time employees, or how the combination of proposed staff meets the PWS paragraphs. This is also an opportunity to explain the benefits of the Offeror’s approach that may be overlooked by simply reviewing the proposed hours.
(iii) VOLUME III – SMALL BUSINESS PARTICIPATION PLAN.
(A) This section will include the Offeror’s Small Business Participation Plan for meeting the requirements of the PWS (limited to 5 pages).
(B) All Offerors (both large and small business) are required to compete a Small Business Participation
Plan. Offerors should propose the level of participation of small business (as a small business prime, joint venture, teaming arrangement, and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goas set forth in the evaluation of this area.
Small Business Participation Level: 10% of the total contract value.
1. Check the applicable size and categories for the PRIME Offeror only – Check all applicable boxes:
[___] Large Prime or [___] Small Business Prime; also categorized as a [___] Small Disadvantaged Business (SBD) [___] Woman-Owned Small Business (WOSB) [___] Historically Underutilized Zone (HUB Zone) Small Business [___] Veteran Owned Small Business (VOSB) [___] Service Disabled Veteran Owned Small Business (SDVOSB)
2. Submit the total combined dollar value and percentage of work to be performed by both large and small businesses (include the percentage of work to be performed by Prime, joint venture, teaming arrangement, and subcontractors):
Total Percentage planned for Large Businesses [___]% = $_____ Total Percentage planned for Small Businesses [___]% = $_____
When combined, Large and Small Business totals must equal 100%
Example: If the Prime proposes a price of $1,000,000 (including all options), and a small business(es) will provide a $250,000 in services/supplies as a prime, joint venture, teaming arrangement, or subcontractor, the % planed for small business is 25%; and 75% for large business, equaling 100%.
Total Percentage planned for Large Business(es) _75%_ = $750,000 Total Percentage planned for Small Business(es) _25%_ = $250,000 100% = $1,000,000
3. Please indicate the total percentage and dollar value of participation to be performed by each type of subcategory small business. The percentage of work performed by Small Businesses that qualify in multiple small business categories may be counted in each category:
SDB [___]% = $_____
HUB Zone Small Business [___]% = $_____
WOSB [___]% = $_____
VOSB [___]% = $_____
SDVOSB [___]% = $_____
Example: Victory Prop Mgt (WOSB and SDVOSB) performing 2%; and Williams Group
(SD, HUB Zone, and WOSB) performing 3%. Results equate to SB 5%; SDB 3%;
HUB Zone 3%; WOSB 5%; VOSB 2%; SDVOSB 2%). SDVOSBs are also VOSBs automatically; however VOSBs are not automatically SDVOSBs.
4. Identify the Prime Offeror and type of service/supply that the Prime Offeror will provide. Then list each of the intended subcontractors and principal supplies/services to be provided by that subcontractor. Provide the CAGE Code, DUNS, and UEI for the Prime and each intended subcontractor. Also, provide the anticipated NAICS code(s) that the Prime Offeror believes best describes the product or services being acquired by its subcontracts with each intended subcontractor. Small business Primes and small business subcontractors that qualify as small businesses in multiple small business categories should be listed in each applicable small business category.
Name of Company Anticipated NAICS Code Type of Service (Include CAGE Code, DUNS, and UEI for each subcontractor) Prime Offeror:
Large Business:
Small Business:
SDB:
WOSB:
HUB Zone Small Business:
VOSB:
SDVOSB:
Example: If a small business qualifies as a WOSB and a SDVOSB, you can add them to each category above in which they qualify.
Notes: i. Pursuant to Sections 8(d) of the Small Business Act, a small business is considered small for Government procurements if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes the product or services being acquired. In other words, the size of the prime’s suppliers is determined by the applicable NAICS code of their joint venture, teaming partner, or subcontract, which may or may not be the same NAICS code as the one for your prime contract with the Government.
5. Describe the extent of commitment to use small businesses (e.g., what types of commitments, if any, are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint venturing, mentor-protégé, etc.). Provide documentation regarding commitments to small business(es) for this effort. Copies of such agreements should be provided as part of your small business participation plan and will not count against the page limitation for this volume.
