Instructions for Submission of Quotation

(Request for Quotation – 140P6219Q0045)

QUOTATIONS WILL ONLY BE ACCEPTED IF SUBMITTED ELECTRONICALLY BY E-MAIL.

ALTERNATE LINE ITEM QUOTES WILL NOT BE ACCEPTED.

1. Documents/Information To Be Submitted With Quotation: At a minimum, the following documents/information shall be completed/provided and submitted with your quotation.

Clauses and Provisions indicated below which are incorporated into the solicitation by reference rather than full text, can be accessed online, in full text, at:

a. Standard Form 1449 / Optional Form 336

(1) If Contractor is offering discount terms, complete Block 12.

(2) Complete Block 17a, also include DUNS #.

(3) Complete Blocks 30a, 30b, 30c.

(4) Enter the Quoted Price on Lines 00010 thru 00050. Enter the GRAND TOTAL of Lines

00010 through 00050 on Page 4 where indicated.

(5) Enter the number of calendar days the quote is valid from the closing date of the solicitation on Page 4 where indicated.

b. Attachment 3 - Technical, Financial & Past Performance Statement - Complete and submit Attachment 3, Technical, Financial & Past Performance Statement along with the additional documents/information that are requested by that Attachment.

c. Refer to ‘Clauses and Provisions’ of Solicitation – FAR 52.212-3 Offeror

Representations and Certifications – Commercial Items -

(1) If Quoter has completed its annual representations and certifications electronically in SAM (at http://www.sam.gov), complete paragraph (b) of the Provision 52.212-3 Offeror Representations and Certifications-Commercial Items, which is found in the solicitation’s Clauses and Provisions. Sign the bottom of the page that contains paragraph (b) and submit with your quote.

(2) Quoter is to identify the applicable paragraphs at (c) through (v) of that provision that the quoter has completed for the purposes of this solicitation only. If any, submit those completed pages with your quote.

d. Refer to ‘Clauses and Provision’ of Solicitation – FAR 52.204-24 Representation

Regarding Certain Telecommunications and Video Surveillance Services or Equipment Complete the representation and submit with quotation package.

e. Refer to ‘Clauses and Provision of Solicitation - Provision 1452.215-71 – Use and

Disclosure of Proposal Information – Department of the Interior (if applicable) https://www.acquisition.gov/browse/index/far http://farsite.hill.af.mil/ http://www.sam.gov/

f. Refer to ‘Clauses and Provisions of Solicitation – Evaluation Factors for Award –

Provide any additional information requested by Evaluation Factors for Award paragraphs (c)(1) through (c)(3) if not called for elsewhere in these instructions or not already included in your completion of SF-1449, and Attachment 3.

g. Additional Information – Provide any other information or documents requested by the solicitation that are applicable to your quotation. See FAR Clause 52.212-1.

h. Amendments - Acknowledgment and submission of all Amendments which may be issued against this solicitation.

2. Point of Contact: The contracting agency’s point of contact for this solicitation is:

3. Amendments to Solicitation: It is the responsibility of the quoters to monitor the beta.sam.gov website to download and acknowledge all amendments issued against the solicitation. https://beta.sam.gov

4. System for Award Management (SAM) and Annual Representations and Certifications: All quoters are to have a Unique Entity Identifier (UEI), formerly known as DUNS, and be registered and active in the System for Award Management (SAM). In accordance with FAR 4.1102(a), offerors and quoters are required to be registered in SAM at the time an offer or quotation is submitted in order to comply with the annual representations and certifications requirements. The Quoter must also have completed their Representations and Certifications and be represented as a small business in the NAICS Code of 238220.

Failure on the part of the quoter to have these requirements fulfilled when submitting your quote will make your quote ineligible for consideration. Access to the System for Award Management (SAM) website can be made at https://www.sam.gov

5. Telephonic Site Visit Brief: A telephonic site visit brief will be conducted on Thursday, September 17, 2020 at 1:00 p.m. central time and shall be approximately 30-45 minutes in duration. To participate, email the Contracting Officer at kathryn_logsdon@nps.gov at least 2 hours prior to the telephonic site visit brief and the call-in number and passcode will be provided. Detailed site visit information is found in Provision 52.237-1 of the solicitation’s Clauses and Provisions.

6. Explanation to Quoters:

a. Quoters may request an explanation or interpretation of the solicitation, or its attachments by contacting the Point of Contact via e-mail named in paragraph 2 above.

b. When sending your question(s), prospective quoters are requested to include the RFQ number and solicitation title in the subject line of the e-mail and the Company’s full name and address within the body of the message. Questions are to be submitted to the POC no later than 2:00 p.m. central time on Tuesday, September 22, 2020.

7. Delivery of Quotations: Quotation packages are to be received by the Contracting Officer no later than Wednesday, September 23, 2020 at 3:00 p.m. central time.

Failure to submit your quotation package in its entirety so that it is received by this time and date may result in the quotation not being considered.

8. Notice of Pre-Award Survey:

a. No award shall be made unless the Contracting Officer can make an affirmative determination of responsibility of the prospective contractor as per the standards identified in FAR Subpart 9.1. Quoters are advised that this is accomplished through a pre-award survey which may require the prospective contractor to provide additional information or documentation as requested by the Contracting Officer. Areas of interest on this survey may include, but are not limited to the following, with regard to the prospective contractor:

(1) Have adequate financial resources to perform the contract, or the ability to obtain them.

(2) Be able to comply with the required delivery date, taking into consideration all existing commercial and governmental business commitments.

(3) Have a satisfactory performance record.

(4) Have a satisfactory record of integrity and business ethics.

(5) Have the necessary organization, experience, accounting and operational controls, and technical skills, or the ability to obtain them, including, as appropriate, such elements as production control procedures, property control systems, quality assurance measures, and safety programs applicable to materials to be produced or services to be performed by the prospective contractor and subcontractors.

(6) Have the necessary production, construction, and technical equipment and facilities, or the ability to obtain them.

(7) Be otherwise qualified and eligible to receive an award under applicable laws and regulations.

b. Quoters are advised that accomplishment of this survey is a part of the determination of responsibility process and is not to be construed that a request to the prospective contractor for additional information and documents is an indication that he/she will receive the resultant award.

9. Compliance: By submitting a quotation, the prospective quoter agrees to comply with all terms and conditions as set forth in the solicitation.
