STATEMENT OF WORK
REPLACING/REPAIR SELECT PORTIONS OF SIDEWALK
Museum of the Plains Indian
INTRODUCTION AND OVERVIEW
The Museum of the Plains Indian (MPI) requires construction services for the removal and replacement of select portions of sidewalk and stoop for the Museum. The construction work shall be based on the approved proposal from the selected vendor. All construction work will adhere to the approved drawings/plans and specifications. The Contractor is responsible for providing all necessary professional and contracted services for the construction of the new concrete sidewalk and stoop. Any additional work outside the original scope must be pre-approved through a formal modification to this award and is contingent based upon the availability of funds.
PROJECT SCOPE
This contract includes, but is not limited to, the removal and replacement of the existing sidewalk and stoop that have heaved or settled, creating a safety hazard for pedestrians. The project requires professional tree removal services and landscaping for removal of trees, roots, and stump. New ¾ inch crushed gravel should be provided; reuse of existing material is not permitted.
OBJECTIVE
The objective of this purchase order to perform site preparation and construction services that include, but are not limited to the following:
• Removal and replacement of existing sidewalk sections
• Removal of designated trees in the project area
• Extraction and disposal of all associated roots and removal of concrete
• Restoration of the affected landscape following tree removal
PERIOD OF PERFORMANCE
The period of performance shall commence on the date of award and conclude 45 days thereafter.
Kick off Meeting
A kickoff meeting will be held via telephone or video conference within 5 business day of award of the contract. The kickoff meeting is an introduction/brief call and to discuss the requirement, schedule, and address any questions.

Museum of the Plains Indian, 19 Museum Loop, Browning, Montana 59417.

REQUIREMENTS
The Contractor shall obtain all required state, local, and tribal permits and clearances for the removal, replacement, and proper disposal of existing concrete materials, as required in the project plans and specification.
The Contractor shall furnish all labor, material, equipment, and supervision necessary to remove the designated stoop and replace select portions of the existing sidewalk, in accordance with project drawings, specifications, and applicable standards.
The scope of work shall include, but is not to be limited to the following task (listed in no particular order):
• shall demolish and remove the selected portions of the concrete as shown on the contract drawings.
• Contractor shall remove concrete to the nearest expansion joint, control joint, or for partial removals, in the location(s) as designated by the Contracting Officer Representative (COR).
• Contractor shall excavate to remove existing base material to subgrade, install and compact new 4” thick aggregate base material in preparation for the placement of new concrete in accordance with the contract documents. Recompact existing subgrade to 95% compaction prior to placing new aggregate base material.
• Contractor shall place 4000 psi concrete (4” minimum thickness) with 6”x6”/W2.9 x W2.9 welded wire fabric (WWF). Install expansion joints, control joints, and provide a broom finish surface to match existing on new concrete surfaces per the contract specifications.
• Contractor shall haul away & dump debris and waste to an approved disposal site.
• Contractor shall provide warranties.
• Contractor shall retain two copies of each submittal for their use and review and provide a copy to the CO and project manager/COR.
• Contractor shall remove three trees, to include all associated roots and stumps, as specified in Attachment 1.
Included are the following attachments showing the area of work:
• Attachment 2 – Overview of Sidewalk Repair and Stoop Area
• Attachment 3 – Stoop Repair Drawing
• Attachment 4 – Stoop Rebar and Repair Specifications
Other Considerations:
The structure will remain occupied and operational during this project. The museum is open to the public Tuesday through Friday, from 9am to 5pm, during the months of June through September. The Contractor shall take pedestrian traffic into account when delineating the construction area. The Contractor must create proper safety closures of those areas until completion of work is done but should not block access to the entrance when possible. If entrance access must be obstructed for the construction work, the Contractor shall coordinate with museum staff in advance to identify an alternate entrance for the public and ensure that clear, visible signage is posted accordingly.
Government Furnished Materials:
The Contractor shall furnish all materials, equipment, and resources necessary to perform the work, as no Government-Furnished Materials will be provided.
Other Services:
The Contractor shall comply with all applicable Codes and Standards adopted by the Department of the Interior.
The Contractor shall comply with the Davis-Bacon Act and ensure that all laborers are paid no less than the applicable prevailing wage rates established by the Department of Labor for the project locale.
The Contractor shall comply with the “Tribal Employment Rights Ordinance” (TERO) and shall pay all applicable TERO fees, tribal taxes, and any other additional applicable local or state taxes, as required
Any changes to the Program of Requirements that impact the building design, construction schedule, or cost shall be submitted by the Contractor to the Contracting Officer Representative (COR), for review and Contracting Officer (CO) for approval.
The MPI will review and approve the contractor’s Construction Schedule prior to the commencement of on-site work.
The MPI will review and approve Contractor’s Quality Assurance Plan and distribute a copy to both the MPI and COR for informational purposes, review and comment.
The MPI will review and approve contractor’s Accident Prevention Plan and provide a copy to the COR.
Construction
General:
The Contractor shall provide all services required to perform the agreed upon demolition and construction work outlined.
Inspections and Quality Controls:
The Contractor shall be responsible for conducting ongoing inspections and quality controls measures throughout the duration of the construction work as it progresses.
At least one full-time inspector shall be employed and shall be available throughout the entire construction performance period. The Contractor shall provide monitoring of the concrete/cement during curing stages due to the high volume of foot traffic 24 hours a day.
The MPI will participate in weekly progress meetings, the final inspection(s) and the punch list inspection(s).
Quality Assurance:
The Contractor shall conduct daily on-site surveillance to verify construction activities are progressing and proceeding as planned and that its inspector and construction management team are appropriately administering the work. The MPI on site staff will also perform periodic checks throughout the construction process, however, the COR shall not direct the Contractor.
Permits:
The Contractor shall secure all required permits.
Construction Management Services:
The Contractor shall provide on-site construction management. The Contractor shall be responsible for monitoring the performance and progress of all construction activities.
Construction Contract Modifications:
The Contractor shall prepare and submit all construction contract modifications requests to the COR for concurrence. The COR will review, and if appropriate, forward the request to the CO for approval and modification execution.
Testing During Construction:
The Contractor shall ensure that the proper material testing is conducted in accordance with industry standards.
Prior to concrete placement, the contractor shall submit concrete mix designs applications to the onsite MPI contact for review and written authorization. The contractor shall also provide delivery tickets (or similar documentation) with each load of concrete delivered to the site, including details of the mix proportions, batching information, and any other information required by the contract or the approving authority. The contractor must obtain written authorization from MPI onsite contact for the approved mix designs before any concrete is placed.
Slump Testing
Concrete for the sidewalk shall be tested for slump in accordance with ASTM C143 (Standard Test Method for Slump of Hydraulic-Cement Concrete). The desired slump range for this project is 4-6 inches or 50-100 mm. Testing shall be performed on fresh concrete immediately prior to placement.
The slump test results, including date, time, location, individual slump measurements, and whether the concrete met the acceptance criteria, shall be recorded in a designated logbook and submitted via email to the museum onsite contact and COR within 24 hours of the test.

The Government requires the Contractor to provide the following deliverables to monitor progress and ensure compliance:
• Daily Status Reports
• Weekly Progress Reports
• Construction Schedule
• Quality Assurance Plan
• Accident Prevention Plan
All reports pertaining to this award should be provided to the COR.
The Contractor shall provide a daily verbal update to the MPI Curator and in her absence the MPI Maintenance Worker.
The Contractor shall provide a brief written weekly progress updates emailed to the COR and MPI Curator.
The COR is not authorized to make changes in the work. Only the CO has the authority to modify changes to the funding or scope of work.
Post Construction Services
As-Constructed Drawings
The Contractor shall prepare and maintain as-constructed drawings throughout the construction process and after construction has been substantially completed. The MPI requires those be provided as PDF format. The Contractor shall provide digital copies to the
COR.
Operation and Maintenance Manuals and Warranties:
The Contractor shall submit both electronic and hard copies of the O&M Manuals and Warranties to the COR. The format and quantity of electronic files shall be determined through mutual agreement between the COR and the Contractor and should be submitted upon completion of the project.
Completion Report:
The Contractor shall prepare a completion report for the project. In coordination with the COR, the Contractor shall agree upon the asset types to be described in the report after award prior to the commencement of work.
The Contractor shall prepare and submit to the COR the “Substantial Completion of Construction Project Report.” A sample format of the report will be provided by the CO for reference.
Substantial Completion shall be defined as the point at which the front entrance is fully constructed and ready for safe public use.
For the sidewalk substantial completion shall be define as the point at which the entire sidewalk section is fully completed and ready for safe public use.
