About this file
This is a Scope of Work (SOW) for optometry services at the McAlester Army Ammunition Plant (MCAAP). The document outlines detailed requirements for providing professional optometric services to approximately 40-50 employees per month, with the contractor's office located within 20 miles of the installation's main entrance. Services include performing basic eye and visual examinations, prescribing and fitting safety glasses and respirator spectacle inserts, and conducting specialized retinal examinations for employees working with lasers.
Key specifications include providing ANSI/ISEA Z87.1 compliant frames with integrated side shields (both plastic and metal options for males and females), polycarbonate lenses, and specific lens options like blue light blockers and progressive bifocals on a case-by-case basis. The contractor must be a licensed optometrist with a doctorate, submit monthly itemized invoices, and complete patient appointments within 30 minutes. This is part of a two-year Blanket Purchase Agreement (BPA) where monthly funding will be determined by the number of employees seen, with invoices processed through a specific administrative workflow involving verification and call order documentation.
