1.0 Objective

Provide a brief description of the Quality Assurance Plan’s intent.

The QAP provides a systematic method to evaluate performance and monitoring for the Noxubee Janitorial Contract. The Contractor is responsible for management and quality control actions to meet the terms of the contract.

All areas scheduled to be cleaned according to the contract scope of work shall be free of dust, debris, smudges, streaks, stains, and unpleasant odors. In addition, the areas shall have an overall appearance of having been cleaned after the janitorial crew has completed the tasks described in the Scope of Work.

2.0 Performance Indicators and Acceptable Quality Levels

Provide a description of the performance indicators to be measured and their acceptable quality levels.

The Contractor shall not miss more than three standards (below) per check.

Refer to the Scope of Work for full details on items to be cleaned under this service contract.

1. Rest Rooms:

Dispensers: Service paper towel, soap and toilet paper dispensers, wipe stall doors, walls and railings. All hardware for dispensers will be closed properly, and locked in their correct positions for operation. If any dispenser should develop a leak, it shall be reported to the COR for repair.

Fixtures: Clean and disinfect all surfaces (inside and out) of toilets, urinals, and washbasins. Clean all surfaces of mirrors, dispensers, and waste receptacles. Clean with pre-approved green seal cleaner. All surfaces shall be free of grime, fingerprints, smudges, soap residue, lint, and dust.

Floors: All tiled floors in the restrooms shall be swept and wet mopped with pre-approved green seal cleaner daily. Prior to being wet mopped, all areas to be mopped shall be swept to move litter, dust and foreign debris from the floor surface. After being mopped, the floor shall have a uniform appearance with no streaks, swirl marks, detergent residue or evidence of soil stains, film, debris or standing water. The Contractor shall spot clean floors when there are noticeable spots.

Insect Debris: Clean/remove spider webs on interior walls and light fixtures.

Paper Products: A full roll will always be kept in the spare toilet paper slot in each dispenser. Paper should be replaced in dispensers in a manner which prevents overloading, sticking together, or fouling the operation of the equipment.

Vertical Surfaces: Clean all tiled wall, door, and partition surfaces.

Vent Covers: Shall be free of dust.

Waste receptacles: Empty all waste baskets and put all bagged trash in the dumpster. Service feminine hygiene disposal stations. New bags (appropriate type for each container) will be placed inside the empty trash receptacles each time the full/used bag is removed. The waxed Kraft paper bag for feminine hygiene disposal stations will be opened, and the edges folded down to ensure it fits correctly within the dispenser’s interior dimensions. Wear disposable gloves when handling these wastes. When contents are spilled or found outside of the bin liner, the bin will be wiped with a disinfectant and dried before putting the new bag in. All hardware for trash receptacles will be closed properly, and locked in their correct positions for operation.

Trash Removal: All trash receptacles are to be emptied. New bags (appropriate type for each container) will be placed inside the empty trash receptacles each time the full bag is removed and taken to the dumpster.

3.0 Surveillance Methods

Provide a description of the evaluations that will be performed based on the indicators in section 2.0.

The COR will conduct random inspections. The Government reserves the right to inspect the Contractor’s performance using 100% inspection and validated customer complaints. When notified of a deficiency, the Contractor shall correct items designated as unsatisfactory or below standard at no additional cost to the Government. All corrective action must be completed within 7 days (i.e. the next weekly service date) of notification of the deficiency; it not, a negative performance rating will be given. Failure of the Government to notify the Contractor of a deficiency does not relieve the Contractor of the obligation to perform services nor does it relieve the Contractor of the responsibility to ensure that quality control measures are followed to ensure contract compliance.

4.0 Incentives

Provide a description of any incentives/penalties.

When janitorial assignments fail to meet the Acceptable Quality Level (missing three or more standards per check), a negative performance rating will be given.
