Addendum to FAR 52.212-1 Instructions to Offerors
Submission of Offers: Offers shall be submitted by the due date and time provided on the SF1449 block 8 to Mindie Dixon at mdixon@usgs.gov.
One (1) electronic copy shall be submitted as follows:
Tab A – Offer Form (Standard Form 1449) - original signature and date; Contract Administration Data; UEI; Acknowledgement of Solicitation Amendments if applicable.
Tab B – Pricing – Utilize Schedule of Services Attachment 3 to provide pricing information.
1. Delivery Time (in Business Days) Confirm the number of business days to deliver results after sample receipt.
2. Price per Sample-Enter the unit price for analyzing one sample.
3. Total Cost- Multiply the Price per Sample by the Quantity listed for each performance period and enter the result in the Total column.
Tab C – Completed copy of FAR 52.212-3 and 52.204-24 if not updated in SAM.gov.
Tab D – Evaluation Factor Responses:
Factor 1: Technical Capability
i. Page Limit is 3 pages.
ii. The contractor’s quote shall provide a technical approach clearly demonstrating how the Government’s requirement will be met. If documentation submitted does not clearly demonstrate the contractor’s ability to provide a service with the stated specifications, the quote may not be considered.
