About this file

This is a Past/Present Performance FACTS Sheet template for offerors responding to the National Mall & Memorial Parks (NAMA) Wayfinding Signs & Installation solicitation (140P1326Q0028) issued by the Department of the Interior National Park Service Intermountain Region.

Offerors must complete the FACTS Sheet by providing detailed information on exactly three (3) relevant contracts of similar size and scope to demonstrate technical capability. For each project, offerors must identify the project title and contract number, period of performance, number of modifications, customer organization, contracting officer and representative contact information, contract type, role (prime or subcontractor), original and final contract dollar values, and National Register of Historic Places reference number if applicable. The submission requires a brief synopsis of effort in no more than two paragraphs (five sentences per paragraph) that highlights work similar to NAMA Wayfinding Signs, followed by a separate paragraph addressing on-time delivery performance. If contract values changed from original to final amounts, offerors must explain the variance in no more than five sentences. The form explicitly requires complete information with no omissions; missing data must be marked as "N/A." Offerors must also indicate whether they have previously manufactured and installed highway signs. The Government reserves the right to independently assess the relevancy of submitted past performance examples and emphasizes that information provided is source selection sensitive and cannot be used for promotional or endorsement purposes.
