About this file

This document is a Past Performance Questionnaire (PPQ) for the Department of Interior's project to repair the Ft. Tompkins Earthen Roof Structure, solicitation number 140P4525R0009. The PPQ is a standardized form designed to collect detailed performance feedback about a contractor's previous project work, with the goal of evaluating their past performance for a potential new contract. The form includes comprehensive rating sections covering relationship with the client, corporate management, quality control, schedule performance, and overall project satisfaction, with ratings ranging from Outstanding (O) to Unacceptable (U). Contractors are instructed to complete Section A with project details and then forward the form to a previous project owner/evaluator to complete Section B, providing detailed performance assessments.

The questionnaire requires the evaluator to rate the contractor on specific performance dimensions, including the relationship between the company and client's contract team, corporate management, integrity, quality control, and ability to meet performance schedules. Key evaluation questions include whether cure notices were issued, if the customer was satisfied with the final product, and whether the evaluator would award another contract to the company. The form allows for an overall rating and provides space for additional comments. Offerors are permitted to submit between 3-5 past performance questionnaires for the solicitation, with projects completed within the past five years, to demonstrate their capability and track record of successful project execution.
