About this file
This is an Indian Affairs Indian Economic Enterprise Representation Form required under the Buy Indian Act (25 U.S.C. 47). The form serves as a self-certification document for enterprises seeking to qualify as an Indian Economic Enterprise (IEE) under Department of the Interior Acquisition Regulation (DIAR) Part 1480.
The form applies to Solicitation 140A2326Q070 for the Enterprise Back-Up Solution, Haskell project. Offerors must certify that they meet the IEE definition at three critical points: when submitting an offer in response to a written solicitation, at the time of contract award, and throughout the full contract term. The form requires offerors to provide the name of the federally recognized tribal entity, the legal business name, the Unique Entity ID (UEI), the name of the 51% (or greater) owner of the IEE firm, and a certifying signature with printed name. Contracting Officers retain the authority to request additional eligibility documentation at any point during the acquisition lifecycle, including pre-award evaluation. False or misleading information submitted during the offer process violates 18 U.S.C. 1001, while false claims during contract performance are subject to penalties under 31 U.S.C. 3729 to 3731 and 18 U.S.C. 287.
