ACTION DETAILS
Program Deficiency
Breaker box that feeds main power to the Montanan house and power to well for outlying areas, not properly secured. If the last screw holding the conduit and box in place fails, could cause fire
Consequence (Severity) I
Likelihood (probability) A
1 - Critical:
Represents an immediate danger to life, health, property, or infrastructure and requires emergency correction or control of the hazard to a lower level of risk.
Initial Abatement Timeframe: As soon as possible within that work shift.
1 - Critical Yes
Recommendation Assign contractor to secure conduit and box appropriately. If temporary fix is used, notify assigned Safety Manager to when permanent fix will take place.
