About this file
This is a Department of Interior Past Performance Questionnaire (PPQ) template used to evaluate contractor performance for the CUGA - Prepare and Paint Headquarters/IT solicitation (140P5326R0026) issued by the National Park Service Southeast Region. The questionnaire serves as a source selection tool and must be completed collaboratively between the offeror and project references.
The questionnaire requires offerors to submit between 2-5 past performance references for projects completed within five years of contract completion. Section A requests basic project information from the offeror, including project title, location, company name, Unique Entity ID/CAGE code, project manager contact information, and contract value (initial and final amounts). Section B is completed by the project evaluator and includes ratings across nine performance categories: client relationship, corporate management and integrity, quality control, schedule performance, cure/show cause notices, contract award likelihood, customer satisfaction, contractor opportunity to respond to negative comments, and an overall performance rating. Ratings use a five-point scale: Outstanding (O), Above Average (A), Satisfactory (S), Marginal (M), and Unacceptable (U). Completed questionnaires must be submitted to the contracting point of contact, Marilia Mateo@ios.doi.gov, by the deadline specified in the solicitation. Late submissions will not be accepted, and offerors should account for electronic transmission delays.
