Attachment 2 The requirements listed in the table below shall be accommodated by the responder offering Sample Management Software. Please indicate Y (Yes) or N (No) if your product meets each requirement in the requirements list below, and use the comments column to further justify how your product meets the requirement.

Ability to manage and fulfil requisitions for Aliquots from internal and external customers

Five (5) standard named users licenses and at least One (1) admin user name

One-time cost validation license

Implementation assistance and one (1) year of customer support with Annual renewal option not to exceed 20% of the software cost

Three (3) days of onsite training
