About this file
The document is a Request for Quote (RFQ) issued by the Indian Health Service (IHS) Phoenix Area Office for Brand Name or Equal Orthopedic and Podiatry Surgical Supplies. The solicitation (RFQ-25-PHX-051) seeks multiple Firm-Fixed Price Blanket Purchase Agreements (BPAs) for surgical supplies from manufacturers including Smith & Nephew, Globus Medical, Arthrex, Treace Medical, Acumed, and Medline Orthopedic, with a 100% small business set-aside under NAICS Code 339113. The procurement will prioritize Indian Small Business Economic Enterprises (ISBEE), Service Disabled Veteran Owned Small Businesses (SDVOSB), or small businesses, with Other Than Small Business quotes considered only if no awards can be made to small businesses.
The solicitation requires offerors to submit detailed pricing information, including manufacturer part numbers, descriptions, unit prices, and percentage discounts by July 28, 2025, at 4:00 PM MST. Proposals will be evaluated using a Lowest Price Technically Acceptable (LPTA) methodology across three factors: Technical (including product documentation, warranty, and delivery), Past Performance, and Price. Delivery locations are Phoenix Indian Medical Center and Whiteriver Service Unit in Arizona, with FOB Destination terms. Electronic submissions must be emailed to Donovan Conley, and offers must remain valid for 120 calendar days. Invoicing will be completed electronically through the Invoice Processing Platform (IPP), and the government intends to award multiple BPA contracts to the most advantageous offerors.

STATEMENT OF WORK
Background:
Phoenix Indian Medical Center (PIMC) provides surgical services to Native American/Alaskan Native patients throughout the Phoenix Service Unit, Phoenix, AZ. PIMC performs an estimated 500 Orthopedic and Podiatric surgical procedures annually. Both the Orthopedic and Podiatry specialties perform surgical repairs of fractures which require specialized instruments and implants. There are over 6,000 implants of various sizes, from multiple venders, which can be stocked in the surgical sets used at PIMC. Their selection and use depend typically on the type of fractures/trauma or bone malformation the patient presents.
Whiteriver Service Unit (WRSU) provides surgical services to Native American/Alaskan Native patients throughout the Whiteriver, AZ area. Whiteriver performs an estimated 100 Orthopedic and Podiatric surgical procedures annually. Both the Orthopedic and Podiatry specialties perform surgical repairs of fractures which require specialized instruments and implants. There are over 6,000 implants of various sizes, from multiple venders, which can be stocked in the surgical sets used at Whiteriver SU. Their selection and use depend typically on the type of fractures/trauma or bone malformation the patient presents.
Objective:
The objective of this multiple award contract (BPA) is to ensure the ready availability of an adequate number of surgical implants of varying sizes and other necessary surgical supplies (biologics/tissue implants) so the PIMC and WRSU can best serve our patient population. The majority of our patients have experienced an injury which requires attention within days. In addition, many of our patients needing these operations come from remote reservations hours away from our facility.
The vendor will provide implants for all Orthopedic and Podiatry trauma cases including large and small fragment instrumentation, as well as mini fragment implants to treat hand and foot surgical cases. Trauma and Reconstructive sets will consist of (Small Fragment locking trauma set, Small Fragment trauma set with medial malleolar hook plate, 1st MPJ fusion set with conical reaming system; Scopion plantar plate repair set; Achilles Speed-bridge tendon repair kit; Midfoot and Rearfoot fusion sets, Rotator Cuff Repair set, Lapiplasty system for Lapidus fusion-(fusion set with bone cutting jigs), External Ring Fixators; Mini Intramedullary Rods, mini Nail or Beams for Charcot reconstruction or joint fusions). These Trauma and Reconstructive sets include various implants systems including plates, screws, pins, and rods. All implants set will exclude spinal trauma and large total joint arthroplasty as Orthopedics does not handle spinal surgery or joint replacement at Phoenix Indian Medical Center (PIMC) or Whiteriver Service Units. Additionally, the Whiteriver SU requests vendors that provide specific implants for elective foot and ankle reconstruction and joint fusions. These sets would include but not be limited to: 1st MPJ fusion, Lapidus fusion (1st TMT joint), Midfoot and Rearfoot fusion sets. Also, requests vendors that provide both: External Ring Fixators such as Taylor Spatial Frames and Mini Intramedullary Rods, mini nails or beams that can be used in the application of Charcot reconstruction or joint fusions.
