QUALITY ASSURANCE SURVEILLANCE PLAN
FOR
PORTLAND DISTRICT (NWP) COMPUTER AIDED DRAFTING (CAD) SERVICES
1. PURPOSE
This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for computer aided drafting (CAD) services. This QASP explains the following:
• What will be monitored
• How monitoring will take place
• Who will conduct the monitoring
• How monitoring efforts and results will be documented
This QASP does not detail how the Contractor accomplishes the work. Rather, the QASP is created with the premise that the Contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations may occur.
2. CONTRACTING OFFICER’S REPRESENTATIVE (COR)
The following personnel will oversee and coordinate surveillance activities.
a. Contracting Officer (KO) - The KO will ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and will safeguard the interests of the United States in the contractual relationship. The KO will also assure that the Contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.
Assigned KO: Theodore S. Blume Organization or Agency: USACE Portland District Contracting Division Telephone: 503-808-4618 Email: Theodore.S.Blume@usace.army.mil
b. Contract Specialist (CS) – The CS is the primary liaison between the Contracting Officer’s Representative (COR) and the KO. The CS is also the second line of contact for the Contractor if the COR is unable to resolve any contract interpretation issues.

Email: Robbie.R.Newland@usace.army.mil
c. Contracting Officer’s Representative (COR) - The COR is responsible for technical administration of the contract and will assure proper Government surveillance of the Contractor’s performance. The COR will keep a quality assurance file. At the conclusion of the contract or when requested by the KO, the COR will provide documentation to the KO. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The Contractor shall refer any changes they deem may affect the contract’s price, terms, or conditions to the KO for action.
Assigned COR: Meghan Garriques, Design Manager Organization or Agency: USACE, Portland District, Hydroelectric Design Center Telephone: 503-808-4410 Email: Meghan.E.Garriques@usace.army.mil
d. Other Key Government Personnel
The COR may assign Quality Assurance Evaluators (QAE) as needed on this contract. The QAE will inspect according to the Contract standards and report back to the COR. The QAE is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The Contractor shall refer any changes they deem may affect contract price, terms, or conditions to the KO for action.
Assigned Primary QAE/TPOC: Eric Holzapfel Organization: CENWP-HDC Telephone: 503-808-4274 Email: Eric.V.Holzapfel@usace.army.mil
3. PERFORMANCE STANDARDS
Performance standards define desired services. The Government performs surveillance to determine if the Contractor exceeds, meets, or does not meet these standards.
The Performance Requirements Summary (PRS) Matrix, Section 6.0, in the Performance Work Statement, includes performance standards. The Government will use these standards to determine Contractor performance and will compare Contractor performance to the Acceptable Quality Level (AQL).

4. METHODS OF QA SURVEILLANCE
The COR is responsible for ensuring the Contractor’s performance is monitored sufficient to determine that the Contract performance standards are met. The PRS matrix above indicates the planned surveillance method for each performance objective. Depending on Contractor performance, the COR may tighten or relax surveillance, as appropriate.
Various surveillance methods exist to monitor performance, including 100% inspection, scheduled and unscheduled inspection, statistical random sampling, and verified customer complaint.
Regardless of the surveillance method, the COR will always contact the Contractor's Contract Representative or Site Supervisor when a defect is identified.
Surveillance results may be used as the basis for actions (to include re-performance, when possible, or payment deductions, when necessary) against the Contractor. In such cases, the Inspection of Services clause in the Contract becomes the basis for the KO’s actions.
5. DOCUMENTING PERFORMANCE
a. Acceptable Performance
The Government will document positive performance. Attachment 1 of this document provides a performance Inspection Checklist template for this service Contract. Any Inspection Checklist or other written quality assurance report may become a part of the supporting documentation for payments or other actions.
b. Unacceptable Performance
When unacceptable performance occurs, the COR will inform the Contractor in writing, normally providing a copy of the corresponding Inspection Checklist. If circumstances necessitate immediate Contractor action, initial contact may be made verbally.
If the COR determines the Contractor cannot or will not re-perform to comply with the performance standard of the PWS, formal communication will be prepared using a Contract Discrepancy Report (CDR) (see Attachment 2) and will be presented to the Contractor's Contract Representative for review and action. The Contractor shall acknowledge receipt of the CDR in writing.
The CDR will specify if the Contractor is required to prepare a corrective action plan to document how the Contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the Contractor has to present this corrective action plan to the COR. The Government will review the Contractor's corrective action plan to determine acceptability.
Any CDRs may become a part of the supporting documentation for Contract payment deductions, or other actions deemed necessary by the KO.
6. RATINGS
Metrics and methods are designed to determine whether performance exceeds, meets, or does not meet a given standard and AQL. A rating scale may be used to determine a positive, neutral, or negative outcome. In general, the following ratings, which mirror the CPARS, will be used:
Exceptional
Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good
Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor was effective.
Satisfactory
Performance meets contractual requirements. The contractual performance contains some minor problems for which corrective actions taken by the contractor are satisfactory.
Marginal
Performance does not meet some contractual requirements. The contractual performance assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains a serious problem(s) for which the contractor’s corrective actions were ineffective.
