About this file

This is an affidavit document required by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking state contract awards or contract renewals. Pursuant to Kentucky Revised Statutes (KRS) 45A.110 and 45A.115, all bidders and contractors must complete and submit a notarized Required Affidavit for Bidders, Offerors, and Contractors to be eligible for contract award or renewal. The affidavit remains valid for one year from the date of execution. An authorized representative of the contracting party must complete the attestation, obtain notarization, and return the completed affidavit to the Commonwealth to maintain eligibility.

The attestation requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that awarding the contract will not violate any provision of those laws. "Knowingly" is defined as the bidder's awareness or should-be awareness of a violation's existence. The Commonwealth retains the right to request an updated affidavit at any time during the contract period. The affidavit must include the contractor's name, address, Commonwealth vendor code if known, signature, printed name, title, execution date, notary information, and the notary's commission expiration date.
