STATEMENT OF WORK

REMOVAL AND INSTALLATION OF POT & PAN WASHING

MACHINES AT

BUILDING 10416

JOINT BASE SAN ANTONIO (JBSA) LACKLAND

1. SCOPE / GENERAL INFORMATION: Contractor shall provide all personnel, equipment, tools, materials, labor, transportation, and supervision necessary to remove old Pot & Pan Washing machines and hardware, provide and install new Pot & Pan Washing machines and hardware, and ensure machines operate properly in the following buildings:

Building 10416 Metzger St, JBSA Lackland , TX 78236

a. All activities shall be coordinated with the Food Service Staff Office and Dining Facility Managers. All work shall be accomplished in accordance with product manufacturer’s recommendations and industry standards, and meet all municipal, state, and federal codes, standards, and guidelines.

The installation time and date must be coordinated and completed NLT 60 days after award.

b. Installation can only be accomplished during non-feeding times (see feeding hours below e-g)

c. Bldg 10416 are open Weekdays for Breakfast 0600-0830, Lunch 1130-1400, Dinner 1700-1930, Saturday and Holiday for Breakfast 0630-0830, Lunch 1130-1330, Dinner 1700-1900 Sundays for Breakfast 0600-0800 Lunch 1130-1330 Dinner 1700-1900

2. DESCRIPTION OF WORK: The contractor shall remove old Pot & Pan Washing machine and hardware. Then install new hardware and new Pot & Pan Washing machine in the facilities identified in Paragraph 1. The contractor shall:

a. Turn off all power to the Pot & Pan Washing machine, and disconnect power from Pot & Pan Washing machine from the facility.

b. Disconnect all plumbing i.e. water lines, drain lines booster water lines.

c. Disconnect all ventilation lines, and any and all soap dispenser.

d. After all connections are disconnected the contractor shall remove the Pot

& Pan Washing machine and place it on the loading dock at the rear of the Dining Facility. The Government will remove the machine and dispose of it from the loading dock area.

e. Place new Pot & Pan Washing machines in the locations where the old machines were, run all plumbing lines, and hardware.

h. Expand/Modify dish table to allow dishes to slide into the dishwasher from the left side.

i. Reconnect all electrical and Soap and other dispensers that were disconnected.

j. Start up the machine(s) ensuring it/they operate as the manufactures guidance states.

k. Ensure water lines do not leak, all drain lines are lined up correctly and drain water goes down the drains per plumbing requirement/standards.

l. Ensure all electrical is wired to meet electrical standards.

3. SAFETY: The contractor shall ensure compliance with OSHA Safety and Health Regulations, prior to daily commencement of work, the contractor shall properly secure the work site and locate and secure tools, equipment, and material as necessary to ensure the safety and protection of personnel, equipment and property.

4. ENVIRONMENTAL COMPLIANCE: The contractor shall be responsible for the removal and disposal of all waste generated from unpacking and installation of these products. The Contractor shall notify the HAZMAT Pharmacy of any hazardous chemicals.

5. HAZARDOUS AND/OR TOXIC MATERIALS. A hazardous material (HAZMAT) is one that poses a risk to the health and safety of the public, and/or the environment if not properly controlled during the handling, storage, manufacture, processing, packaging, use, disposal, or transportation. Hazardous material may be elements or compounds, and can be found as gases, solids, or liquids. HAZMAT is defined under Section 1910.1200© of Title 29 of the Code of Federal Regulations.

5.1 Contractor shall identify and report HAZMAT usage to the HAZMAT Pharmacy according to local procedures. The contractor shall submit a completed AF Form 3952, Chemical Hazardous Material Request/Authorization listing all HAZMAT that will be used in performance of the contract. Additionally the contractor is required to submit Material Safety Data Sheets (MSDS) for each of the listed materials in accordance with safety regulations.

5.2 The phone number for the JBSA Lackland Hazardous Material Pharmacy is 671-5389. A direct Point of Contact will be provided upon contract award.

6. WORKMANSHIP AND REGULATORY COMPLIANCE. This is an occupied building.

Materials on site must be kept in such a manner that continuing working operations is not impaired. The contractor shall take precautions to prevent damage to government property. The contractor shall repair or replace damaged items at no additional cost to the government. All analysis, workmanship, and quality control performed under this contract shall conform to published and recognized authoritative sources of good industry standard and practices in compliance with OSHA Standard 1926 Subparts H and O. The contractor will be responsible for any additional cost required to insure compliance to said standards and practices.

7. WASTE DISPOSAL AND ENVIRONMENTAL PROTECTION. At the end of each work day, the contractor shall remove all residual equipment, components, materials, and /or debris generated and disposed of in appropriate government containers. All areas shall be cleaned of debris at the end of each working day and prior to final acceptance of the work.

8. SECURITY AND CLEAN-UP. All facilities shall be secured at the end of each day. The contractor shall ensure that all work accomplished is limited to the terms and conditions outlined in this SOW. The contractor shall coordinate with the government representative(s) designated during the pre-con upon arrival and prior to leaving the work site each day. The government representative(s) will be located in building 7065, Suite 228.

9. WARRANTIES. Installation shall be warranted from defect for one year, and all standard system warranties shall apply. Warranty shall begin the day of completion and acceptance by 802 Force Support Group.

10.RECORDS MANAGEMENT. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW the AF Records Management procedures in Air Force Instruction (AFI) 33-322, Air Force Manual (AFMAN) 33-363 and Air Force Instruction (AFI) 33-364 (or their subsequent replacements)

11. COMPLETION OF WORK. The contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all equipment and systems shall be fully operational at the completion of work for this project. All surfaces shall be cleaned; tiled floors swept & mopped.

II. GENERAL INFORMATION

1. POINTS OF CONTACT. Upon award, the contractor will provide to the contracting officer a list of all contract personnel who will be performing services on this contract.

2. HOURS OF WORK: Normal hours of work are 07:30 AM to 4:30 PM, Monday thru Friday, except government holidays.

3. GOVERNMENT FURNISHED SERVICES.

3.1 UTILITIES. The government will provide utilities (water and electricity) and emergency, fire and security police response services.

3.2 POLICE PROTECTION. The 802d Security Forces Squadron shall provide security and law enforcement response and service to the contractor.

3.3 FIRE PROTECTION. The Lackland Fire Departments shall provide initial response, EMS, and fire protection services to the contractor.

4. TRAFFIC REGULATIONS. The contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. The contractor’s personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hand-free phones, is prohibited while driving on military installations in San Antonio, TX. (See attached contract clauses LAFB 0009-Cell Phone and LAFB 0010-Securtiy Requirements for solicitations and contracts.)

5. QUALITY ASSURANCE. The government will evaluate the contractor’s performance during various phases of the project. Quality assurance maybe performed randomly by 802d TRW/SE (Wing Safety), 802d FSS/FSVF (Food Service), and /or the contracting officer and representatives. They may observe performance to monitor quality of work, insure cleanliness, and compliance with safety and security procedures. At the time of award, the contractor shall designate a program manager who will be available throughout the project to discuss with the government inspectors any quality issues that may arise.

6. INSPECTION AND ACCEPTANCE. All services performed under this SOW are subject to inspection by the government at the completion of each task. The contractor, at no additional expense to the government, shall promptly correct unsatisfactory services.
