Statement of Work (SOW) Project: Building 20 Clinic Floor Replacement

1. GENERAL INFORMATION

1.1. Background: The USDA, NCAH at 1920 Dayton Avenue, Ames, IA has an onsite clinic within Building 20. The carpet tile floor in the hallway, waiting room, and administrative areas of this clinic is aging and needs replacement. This project will replace the carpet tile flooring with vinyl composite tiles in these areas.

1.2. Scope of Work: This work consists of removing existing vinyl wall base plates and carpet floor tiles, preparing the surface, and installing vinyl composite tiles into the clinic’s waiting room, nurse’s office, file room, and hallway. New base plates shall be installed and transitions between floor areas added.

1.3. Note: Every effort has been made to depict this work in the attachments; however, the content included is approximated. Interested parties are strongly encouraged to visit the site of the proposed work and review the existing conditions there relating to construction and labor, and to be fully informed as to the facility involved, and the difficulties and restrictions that may affect the performance of the Contract.

Failure to visit the site will not disqualify those submitting a proposal; however, change order requests for items that could have been reasonably ascertainable from a site visit (and included in a proposal) will be reviewed by the CO and evaluated accordingly.

2. CONTRACTOR REQUIREMENTS

2.1. Technical Requirements/Tasks:

The contractor shall provide the labor and materials for the following:

2.1.1. Carpet Removal and Floor Preparation: Contractor shall remove the existing 4” vinyl wall base and carpeted floor tiles in highlighted area of work (see attached drawings), any associated pad, and all adhesive down to bare/clean floor to prepare for installation of new vinyl composite floor tiles, new 4” vinyl wall base, and new transitions.

2.1.1.1. Removal of existing carpet tiles under built-ins shall be cut plumb such that, upon installation of tile and wall base, there is no indication of carpet.

2.1.1.2. Floor preparation shall be in accordance with the attached specification (Section 09650) and the manufacturer’s recommendation.

2.1.1.3. All demolished materials shall be removed from the campus and disposed of appropriately by the contractor.

2.1.2. Vinyl Composite Floor Installation: Contractor shall install new vinyl flooring for areas marked on the attached PDF.

2.1.2.1. Installation shall be in accordance with the attached specification (Section 09650) and the manufacturer’s recommendation.

2.1.2.2. Tiles shall be vinyl composite tile by Armstrong. Model shall be 12” x 12” Standard Excelon, Imperial Texture, model 51904

2.1.2.3. Contractor shall provide physical sample to Owner for verification prior to purchasing.

2.1.3. Wall base Installation: Contractor shall install new 4” vinyl wall base/moulding along locations of new flooring and affixed to built-ins to match original placement.

2.1.3.1. Installation shall be in accordance with the attached specification

(Section 09650) and the manufacturer’s recommendation.

2.1.3.2. Wall base shall be 4” vinyl wall base manufactured by Roppe or approved equal (snow color).

2.1.3.3. Contractor shall provide physical sample to Owner for verification prior to purchasing.

2.1.4. Transitions: Provide transitions where flooring termination is higher than the adjacent finished flooring and at transitions between different flooring materials.

2.1.4.1. Installation shall be in accordance with the attached specification

(Section 09650) and the manufacturer’s recommendation.

2.1.4.2. Contractor shall provide physical sample to Owner for verification prior to purchasing.

2.2. General Requirements

2.2.1. The Contractor shall furnish all labor, materials, equipment, supplies, transportation, supervision, safety equipment*, and incidentals for all elements of the work to provide a complete and properly functioning installation in accordance with this Statement of Work.

* Note: Contractor is solely responsible for safety measures and conformance to applicable requirements in accordance with FAR clauses referenced in Solicitation package

2.2.2. The scope of the project shall be performed in a neat, workmanlike manner and shall be sequenced for efficient workflow and to minimize interruption to the daily activities and ongoing research at the facility to the greatest extent possible.

2.2.3. Contractor shall provide all means and methods to protect the facility and its installations from all project activities, including protecting the facility from environmental elements as the improvements are completed.

2.2.4. The Contractor shall submit a safety plan to the USDA no less than 10 days prior to the commencement of work.

2.2.5. The Contractor shall submit a quality control plan no less than 10 days prior to the commencement of work.

2.2.6. All work will be performed by qualified personnel.

2.2.7. Prior to commencing work, the general contractor shall provide proof that an OSHA certified “competent person” (CP) (29 CFR 1926.20(b) (2) will maintain a presence at the work site whenever the general or subcontractors are present.

2.2.8. Anything mentioned in the Scope of Work and not shown on the drawings or shown on the drawings and not mentioned in the Scope of Work, shall be of like effect as if shown or mentioned in both.

2.2.9. The finished product and all of its components shall provide a complete operational and functional system per this Scope of Work and the manufacturer’s specifications.

2.2.10. Waste Disposal:

2.2.10.1. Contractor shall submit documentation on the amount of project waste diverted to recycle as part of project closeout. Contractor shall submit the type and amount of waste that was diverted to recycle, present information in a clear, typewritten format.

2.2.10.2. If no project waste is diverted to recycle, the Contractor shall submit a letter stating this and why diversion was not practical.

2.3. Scheduling and Coordination:

2.3.1. The clinic must remain operational to respond to an emergency during this project.

2.3.1.1. A temporary walkway/passageway shall be available on site for immediate use whenever the waiting room and hallway become unsafe for emergency foot traffic.

2.3.2. Floor replacement may not occur between the dates of November 1, 2024, and March 31, 2025.

2.3.3. Some clinic personnel are expected to be out in October 2024. The strongly preferred dates of completion are during this time, from 10/13/2024 to 10/18/2024. Onsite facilities staff would like to return items into place on 10/21/2024 so that normal clinic services must resume on 10/22/2024. Coordination with the COR will be necessary to ensure all furniture can be removed and replaced by on-campus personnel prior to and after completion of works.

2.3.3.1. The clinic must not be without operations for more than 5 business days. Room accessibility importance is as follows: the hallway and lobby area followed by the office area then file room.

2.3.4. Schedule shall be designed to minimize total clinic downtime (including furniture moving efforts by campus staff). This will require advanced coordination and indication of planned moving dates on contractor’s schedule.

2.4. Special Provisions

2.4.1. Do not disturb portions of site beyond areas which the work is approved.

If land is disturbed due to staging or other reasons, the contractor is required to returns the condition back to pre-construction condition or better at no cost to the government.

2.4.2. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer’s Representative. At the completion of operations, existing work must be in a condition equal to or better than that which existed before new work started.

2.4.3. Keep driveways, entrances, walkways, and doorways serving premises clear and available to the Government, employees, visitors, and emergency vehicles at all times. Do not use these areas for parking or storage of materials.

2.4.4. Any damage resulting from contractor's operation shall be brought to the attention of the Contracting Officer's Representative immediately.

2.4.5. Finished surfaces of existing facilities that are affected, marred, scratched, or damaged by the contractor shall be refinished to match original condition.

2.4.5.1. Patching material shall be of the same type, style, and thickness, and shall be finished to match existing adjacent surface construction in color and texture.

2.4.5.2. Patching material, paint color, paint material and paint application methods shall be approved in advance by the Contracting Officer’s Representative.

2.4.6. No building structural members shall be cut or altered without prior approval of the Contracting Officer's Representative.

2.4.7. Material and application methods shall be approved in advance by the Contracting Officer’s Representative.

2.4.8. The contractor shall take all necessary and reasonable measures to keep the work site clean of trash and debris. Work areas shall be cleaned immediately as work in that area is completed, and before moving to a new work area. Place all trash and debris in OSHA approved containers, remove, and dispose of trash off site daily.

2.4.9. The contractor shall move all materials, and equipment, including ladders and scaffolding from the site at the completion of each day's operation unless otherwise approved by the Contracting Officer's Representative.

2.4.10. The contractor shall provide proper safety equipment to workmen and ensure a safe environment for employees and the general public at all times. The contractor shall work to ensure free, unobstructed, and safe access to and about the work area.

2.4.11. The Contractor shall ensure that any materials that are delivered by FedEx, UPS, etc. are delivered to the jobsite and NOT to the USDA warehouse dock at Building 24 or to the north warehouse, Building 11. The USDA and/or the COR will not be responsible for receiving any materials destined for the project. USDA reserves the right to refuse any unscheduled deliveries that arrive at either warehouse or at the main gate.

3. PRODUCT SPECIFICATIONS

3.1. Basis-of-design products are indicated in this SOW to convey a specified standard of quality and salient characteristics desired but are not to be considered a single-source specification. Preferred products include:

i. 4” vinyl wall base manufactured by Roppe or approved equal (snow color).

ii. Vinyl composite tile by Armstrong. Model shall be 12” x 12” Standard Excelon, Imperial Texture, model 51904.

It shall be the Contractor’s burden to furnish all documentation and / or samples to show equality with specified items. Owner’s decision on equality is final.

Manufacturers / products that are submitted as “Approved Equal” are reviewed during the shop drawing process, not during bidding.

4. REGULATORY REQUIREMENTS

4.1. The equipment materials, installation, workmanship, and operation shall conform to the most stringent applicable requirements of the most recent editions of the associated publications, in the below section and in the attached specifications unless otherwise noted:

iii. National Fire Protection Association (NFPA)

1. NFPA 101 – Safety to Life from Fire in Buildings and

Structures.

2. NFPA 1 – Uniform Fire Code

3. NFPA 5000 – Building Construction and Safety Code

iv. Underwriters Laboratories, Inc. (UL)

v. Appropriate UL Standards

vi. State and Local Building Codes as adopted by the Authority Having Jurisdiction (AHJ).

vii. The Uniform Federal Accessibility Standards (UFAS)

viii. Occupational Safety and Health Administration (OSHA)

5. INTENT OF CONTRACT DRAWINGS

5.1. Government provided drawings, henceforth referred to as “Contract Drawings,” do not attempt to show complete details of building construction that may affect installation. Government provided diagrams of proposed systems are for reference only and do not necessarily show physical arrangement of all equipment and/or maximum number of devices. Minimum system requirements are shown.

5.2. Locations of existing and Government concept devices are approximate and may be distorted for clarity in representation.

6. WORK RESTRICTIONS & CONDITIONS

6.1. Site Security Access

6.1.1. This project construction site is located inside building 20’s clinic. An NCAH escort is required in the clinic space unless otherwise stated.

6.1.2. Access to the construction site is controlled by a security fence and visitor/contractor check-in/check-out system. All contractor/subcontractor employees shall sign-in and out at the main entrance gate and wear a contractor/visitor identification badge at all times while on the site. The ID badges will be turned in at the end of each workday. A current government-issued identification with a picture ID is required for entry.

6.1.3. A minimum of 5 days of advanced registration is required for all personnel entering the site that are foreign nationals and 3 days for all other workers.

6.1.4. Construction vehicles and a mobile construction trailer will be allowed access adjacent to the work; contractor employee’s personal vehicles shall be parked in the facility parking lots. Appropriate vehicle badges shall be displayed at all times while the contractor is on site.

6.1.5. The Contractor shall use only the service roads and access routes approved by the Contracting Officer’s Representative.

6.1.6. Unless accompanied by a NCAH/USDA employee, the Contractor, subcontractors, and their employees shall not enter buildings or facilities not specifically part of this project. Reasons included, but are not limited to, disease control and health requirements.

6.1.7. The Contractor shall adhere to all Site, ARS, APHIS and USDA security policies and procedures including, but not limited to, Code of Federal Regulations Requirements, Standard Operating Procedures, Security Directives, Security Procedures, General Orders, Post Orders and any applicable Government Policies and Procedures.

6.1.8. Additional security requirements may be imposed when the Federal Government raises the Homeland Security Alert Level. Additional directions will be provided if the situation occurs.

6.1.9. No pets or animals are allowed inside the Security Gate, even if left in vehicle.

6.1.10. No firearms or weapons are allowed on site.

6.1.11. Failure to comply with access requirements shall be adequate grounds for removal of the Contractor/subcontractor employees from the site and barring further site access to individuals.

6.2. Work Hours: Regular work hours for the site are 8:00am to 4:30pm, Monday through Friday, excluding Government holidays. Work scheduled outside regular work hours including late hours, Saturdays, Sundays, and Government holidays requires Contracting Officer’s Representative (COR) approval in advance. Make application to the COR at least 5 calendar days prior to such work to allow arrangements to be made by the Government for access to the work area and inspection & monitoring arrangements. Requests shall include the exact dates, hours, location, and purpose of the exception. If approved, the COR will coordinate access with the site security office.

6.3. Project Timeline: Contractor shall be given 90 days from the “Notice to

Proceed” to complete the project. Disruption to the clinic shall be minimized.

Coordination is required from the USDA’s facilities engineering to move existing furniture and equipment from the site. The schedule will be refined during the initial meeting.

7. TEMPORARY FACILITIES

7.1. Utilities: The Contractor may use utilities within Building 20’s clinic at no cost to the contractor but will be responsible for making and removing any utility connections.

7.2. Telephones: The Contractor shall equip all his foremen and work leaders with cell phones as necessary to coordinate the work effort. The Government will not provide any phone service to the project.

7.3. Construction Trailer: A mobile construction trailer will be allowed for this project. Its location must be coordinated with the Contracting Officer's Representative.

7.4. Toilet Facilities: Contractor may use facilities available in the lobby of building 20.

8. COORDINATION

8.1. The contractor shall coordinate all work with the Contracting Officer's Representative.

8.2. The contractor shall submit an installation schedule for review and approval by the Contracting Officer's Representative within ten (10) days of task order award.

8.3. The contractor shall be solely responsible for coordinating work with all subcontractors and ensuring that all items of joint use in conjunction with installations are completed.

9. SUBMITTAL & SHOP DRAWINGS PROCEDURES

9.1. Submittal Register: At the beginning of the project, the Contractor shall provide a Submittal Register identifying all the items called out in the Specifications for submittal to the COR for approval. The Contractor shall update and maintain the Submittal Register to show the current status of all required submittals.

9.2. Submittal Requirements:

9.2.1. Format: There is no formal submittal transmittal or format for this project other than the documents shall be submitted as PDF files, scanned, or created in documents that can be easily read when printed on 11x17 sheets. Exceptions for larger sheet sizes will be considered by the COR.

Failure of the Contractor to provide legible submittals will be adequate grounds for rejection.

9.2.2. Identification: Submittals shall be labeled and marked for this project.

The identification shall include Contractor/sub-contractor’s name, date of submittal, description of item, specification section, and details on product data that are marked clearly for this project.

9.3. Submittal Procedures: All submittals will be posted in PDF by the Contractor and sent to the USDA for review. This same submittal process will be used for RFIs, Change Bulletins, etc.

9.4. Approved Submittals: The approval/acceptance of submittals is not to be construed as a complete check. Approval or acceptance will not relieve the Contractor of the responsibility for any error which may exist.

9.5. Submittal items for this project:

• Vinyl composite tile material

• 4” vinyl wall base materials

• Transition materials

• Adhesives: Manufacturer's descriptive data, documentation stating physical characteristics, and mildew and germicidal characteristics.

Material Safety Data Sheets (MSDS) for all primers and adhesives shall be provided to the Contracting Officer’s Representative. Highlight VOC emissions.

10. RECORD DRAWINGS

10.1. Redline Specifications: During the construction period the Contractor shall keep an updated & marked-up or “redline” copy of the project specifications on the jobsite. The redline set shall include notes on changes, RFI responses, and other field notes. At the end of the project the Contractor shall electronically scan the redline specifications and provide an electronic copy in PDF to the Owner along with the close-out documentation.

10.2. Redline Drawings: During the construction period the Contractor shall keep an updated “redline” or “conformed” copy of the project drawings on the jobsite. The redline set shall include notes on changes, RFI responses, and other field notes. At the end of the project the Contractor shall electronically scan the redline drawings at full-size and in color and provide an electronic copy in PDF to the Owner along with the close-out documentation.

11. WARRANTY DOCUMENTATION

11.1. Warranty Duration: Provide a minimum of a one-year warranty for labor to repair of faulty workmanship and material. The warranty shall start at the completion of the project. Response time shall be less than 5 days.

11.2. Warranty List: Provide summary sheet in spreadsheet format with all the warranties listed by section, description, supplier, warranty contact, start date, end date. Provide electronic PDF file to the COR.

11.3. Warranty Copies in Binder: Provide the COR one signed & dated copy of all warranties for products and equipment installed on the project. An electronic (scanned) copy of the Warranty Binder in a PDF file shall be submitted with the final close-out documentation.
