Elevator Maintenance
FAR 52.212-1 Addenda:
1. Instructions to Offerors
a. The Government reserves the right to revise or amend the performance work statement (PWS) or the solicitation, to include any and/or all attachments, prior to the quotation closing time. Such revisions will be communicated by amendment to this solicitation. If such amendment require material changes in quantities or prices, the quotation closing date may be postponed by enough days to enable offerors to revise their quotations. In such cases, the amendment will include an announcement of the new proposal closing date and time. Amendments will be posted on the Internet with the solicitation documents without notice. Offerors are responsible for accessing all amendments prior to quotation submission at the following website: beta.SAM.gov.
b. Questions All questions must be submitted within ten (10) calendar days of issuance of this request for quote.
2. Proposal Preparation Instruction
a. To assure timely and equitable evaluation of quotations, offerors must follow the instructions contained herein. Quotations must be complete, self-sufficient, and respond directly to the requirements of this solicitation. A complete quotation consists of executed quotation documents; for example, this includes but is not limited to completing proposed unit prices with net amounts for all subcontract line item numbers (SLINs) 0001AA – 4002AA AND completion of FAR 52.212-3 for FAR and DFARS reports in the System for Award Management (SAM) at www.sam.gov no later than the date and time set for closing. Failure to furnish a complete quotation may result in the quotation being determined incomplete and ineligible for award by the government. Quotations shall be submitted via electronic or hard copy (e-mail or compact disc only – USBs will not be accepted) prior to the time set for proposal closing to:

e. NOTICE
Real ID Act compliance is being enforced at Malmstrom AFB. Offerors are cautioned that Malmstrom AFB has stringent visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass and be cleared by the Government point of contact to be visited prior to entrance. It is possible that the point of contact may not be available to clear offerors for entrance which may prohibit submission of hand carried offers; consequently, offerors electing to hand carry their quotations for submittal just prior to quotation closing time

f. General Format and Content
The offeror shall submit the following volumes of material. A complete quotation consists of the following *volumes:

*Each volume shall be submitted in either one hard copy or one electronic copy.
** Offerors shall complete Sections 1 through 3 of Attachment 5 for three references.
g. Pages exceeding the page limitations stated within this document will not be read or considered in the evaluation, and will be removed from the proposal. A page is defined as one face of an 8½” x 11” sheet of paper containing information. Pages shall be numbered consecutively within each volume. Font shall be no less than Time New Roman 10 pt. Line spacing shall be no less than double-spaced. Pages shall be numbered. The Technical information shall be submitted in either MS Word or PDF format.
h. To facilitate review and evaluation for this solicitation, the Government proposes to transmit the data via commercial systems. Distributed material will be identified as source selection sensitive and distribution strictly limited to appropriate personnel. Should any offeror object to their proprietary information being shared between source selection evaluators via commercial e-mail as described above, please advise the Contracting Officer, Jayce Blood, at 341st Contracting Squadron, 7015 Goddard Drive, Malmstrom AFB MT 59402, telephone 406-731-3625.
i. A Question and Answer period of ten (10) calendar days beginning on the initial date of solicitation will be allotted for all prospective offerors. Please note, questions will not be answered after the 10 day period.

FAR 52.212-2- Addenda:
This section outlines the process used to evaluate each quotation. If at any point an offeror’s quotation is determined incomplete or unacceptable and/or receives an “unacceptable” rating the evaluation of that offer may cease and that offer may not be considered for award. A quotation receiving a “satisfactory,” or better rating for technical capability and “neutral,” “satisfactory,” or better rating for Past Performance, either initially or as a result of interchanges, may be considered for award. Offerors are reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without interchanges.
a. The evaluation process shall be conducted as follows:
1. Completeness – does the offer conform and provide all requirements outlined in the solicitation. This includes, but is not limited to, completing of FAR 52.212-3 Representations and Certifications FAR and DFARS reports in the System for Award Management (SAM) at www.sam.gov. Failure to provide and/or complete all requirements may be grounds for an incomplete determination and render the offer ineligible for further consideration/evaluation.
2. All complete offers will be sorted in order from lowest to highest based on the Total Evaluated Price (TEP).
3. The offeror with the lowest TEP will be evaluated under Technical Capabilities. If the lowest offeror receives a technical Rating of “Exceptional,” the Government will cease technical capability evaluations and move on to past performance evaluations. If the lowest TEP does not meet this standard then evaluation will continue until all offerors are evaluated or the next TEP meets the identified requirement above.
4. The Government will evaluate past performance of all offers evaluated under technical. If a rating of “exceptional” is not earned, technical and past performance evaluations will continue until an offeror receives the criteria of “exceptional” for both categories and all offerors are evaluated.
5. In the event the low offeror meets the technical capabilities rating of “Exceptional” and past performance rating of “Exceptional,” the Government will review the two subsequent low TEP offerors to ensure competitive pricing. Price evaluation will document the fairness and reasonableness of the TEP of all offerors evaluated.
6. The Government will conduct an integrated assessment of the complete, acceptable offers to determine best value on a minimum of three (3) offerors, unless fewer are received. A best value determination will be made considering price, technical capabilities and past performance. This may result in award to a higher rated offeror.
B. Price
1. The Government will calculate the TEP by:
a. The offeror’s total proposed price will be determined by multiplying the quantities identified in the SF 1449 by the unit price, limited to two decimal places for each SLIN, to confirm the extended total amount for each SLIN. The sum of all extended amounts will be the offeror’s total proposed price.
b. Regarding SLIN X002AA, the Government will make award for this SLIN at NTE $10,000.00, which will be issued via work order. The Government will provide the awardee a copy of a work order form. However, offerors SHALL provide the fully burdened hourly rate for their technicians under SLIN X002AA. For evaluation purposes the Government will utilize 20 hours as the unit.
c. In order to meet the requirements of FAR Parts 6 and 17, the government will evaluate the option to extend services under FAR Clause 52.217-8 by adding one-half of the offeror’s final option period prices to the offeror’s total proposed price. Offerors are required only to price the base and option SLINs. Offerors shall not submit a price for the potential six-month extension of services period.
d. The offeror’s Total Evaluated Price (TEP) will include the base period and all option periods.
This will not include the potential six-month extension of services period.
e. Price Reasonableness: The techniques and procedures described under FAR 13.106-3 will be the primary means of assessing the quotation’s reasonableness.
C. Technical Capability
1. The Offeror’s Technical Capability shall consist of a written narrative describing the Offeror’s technical, managerial, certification, and mobilization approach for meeting the requirements of the PWS. This should demonstrate the Offeror’s ability to perform the technical requirements.
The written narrative shall consist of the following:
a. Qualifications/Certifications of Technician(s) Current Montana Elevator Licenses or Plans to Obtain. Montana elevator licenses include inspectors, mechanics, and contractors. If current Montana elevator licenses are not obtained the Offeror will provide a plan to acquire the applicable licenses. Note, if a mechanic is a sole proprietor, the mechanic must attain a contractor license.
b. Operations Plan
Offers shall detail how they intend on mobilizing to the sites. Said mobilization must include a narrative of how the offeror intends to meet the requirements of emergency calls detailed at PWS para. 5.2.
2. The Government will review the Offeror’s proposed approach to verify it will result in successful contract performance, and will assign a rating to the technical proposal. The technical definition ratings are as follows:
Factor Description Exceptional Technical Capability meets contractual requirements and exceeds many to the Government's benefit. The elements being assessed identify limited risk.
Very Good Technical Capability meets contractual requirements The elements being assessed identify some risk.
Satisfactory Technical Capability meets contractual requirements.
The element being assessed contains risks deemed acceptable.
Unsatisfactory Technical Capability does not meet most contractual requirements and recovery is not likely in a timely manner.
D. Past Performance Information
1. The Government will evaluate Attachment 5 Past Performance Questionnaire in accordance with FAR 13.106-2(b)(3). For the purpose of this evaluation, “recent” is defined as performance occurring within the last three (3) years from the date of release of this solicitation. References determined to be not recent or relevant will not be evaluated.
2. The following relevancy definitions apply:
Rating Description Relevant
Past performance of elevator maintenance services provided within the PWS of multiple elevators that are serviced annually.
Not Relevant
Past performance of elevator maintenance services that do not meet the elements of the PWS.
3. The following performance definitions apply
Factor Description Neutral Neither favorably nor unfavorably.
Exceptional Performance meets contractual requirements and exceeds many to the Government's benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Marginal Performance does not meet some contractual requirements.
The contractual performance being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
Satisfactory Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.
ii. If the offeror is only able to provide one reference within Attachment 5, references two and three will receive a rating of “Neutral.” Neutral is no performance record is identifiable or the offeror’s performance record is so sparse that no confidence assessment rating can be reasonably assigned. If an offeror provides more than three (3) references, evaluation of references will stop after reaching the third, chronologically listed reference.
iii. The Government may use present/past performance information obtained from offerors’ references, other sources such as other federal government offices, Past Performance Information Retrieval System (PPIRS)/Contractor Performance Assessment Reports (CPARS), private and commercial sources, or any combination thereof. Consideration will be given to whether the present/past performance information is applicable to the prime contractor, teaming partners, and/or joint venture actually performing that part of the requirement. This information may include efforts performed by other divisions, critical subcontractors, or teaming contractors, if such resources will be brought to the current effort.
iv. The Government will evaluate all present/past performance information submitted by the offeror notwithstanding the paragraph above. Offeror’s will be offered the opportunity to respond to negative present/past performance reports if they have not had a previous opportunity to do so.
