Addendum to 52.212-1 – Instructions to Offerors-Commercial Items

The provisions outlined in this addendum are in addition to the requirements stated in 52.212-1. If there is a conflict between the provisions of this addendum and the requirements in 52.212-1, the provisions in this addendum will take precedence.

Submission of Offers

A. The proposal submitted in response to this solicitation must be in compliance with the requirements stated in the solicitation, associated attachments, and Performance Work Statement (PWS). All claimed capabilities to meet the requirements shall be realistic and are subject to verification by the Government. Non-conformance with these instructions may result in rejection of the proposal, an unfavorable proposal evaluation, or being deemed ineligible for award.

B. Offerors shall clearly identify any exception to the solicitation terms and conditions and shall provide complete supporting rationale. The Government reserves the right to determine any such exceptions are unacceptable. Any exception determined unacceptable by the Government may result in the Offeror’s proposal being determined unacceptable and ineligible for award. If no exceptions are taken, include a statement stating so within the proposal cover letter.

C. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but, rather, shall provide convincing rationale to address how the Offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities or experience and will base its evaluation on the information presented in the Offeror's proposal.

D. Discrepancies. If an Offeror believes the requirements in these instructions contain an error, omission, or are otherwise unsound, the Offeror shall immediately notify the Contracting Officer (CO) in writing with supporting rationale as well as the remedies the Offeror is asking the CO to consider as related to the omission or error.

E. The Government reserves the right to award without interchanges or to make no award at all depending upon (1) the quality of quotes received and (2) whether proposed prices are determined to be fair and reasonable.

F. This acquisition is 100% set aside for Economically-Disadvantaged Women-Owned (EDWOSB) under North American Industry Classification (NAICS) Code: 621112, Small Business Size Standard:

$12M.

G. Each Offeror shall submit a proposal electronically to the email box of the Contract Specialist (CS), Jacob Borsh at jacob.borsh@us.af.mil, and the Contracting Officer (CO), Arthur Morales at arthur.morales@us.af.mil. The proposal (via email) shall be submitted no later than the required submission due date specified below. For the electronic file, the Offeror shall indicate part number and title as described in the table in paragraph I below, and use separate files to permit rapid location of information. The Offeror shall submit a proposal for consideration no later than 4:00 pm (Mountain), 17 March 2021. Only one (1) proposal shall be submitted per company in response to this requirement. Proposals received after the date/time specified may not be evaluated. Questions regarding this solicitation shall be submitted through email to the CS and CO no later than 12:00 pm (Mountain), 9 March 2021.

H. To assure timely and equitable evaluation of the proposal, the Offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I

– Cover Letter/Technical Capability and Part II – Price.

I. All proposal information shall be submitted in either MS Word (readable by MS Office 2016) or PDF format. Each part shall be submitted as a separately- labeled electronic file. Text shall be single spaced, on an 8 ½ x 11 inch sheet with a minimum one-inch margins all around. Font shall be no less than Times New Roman 10 pt. or no more than Times New Roman 12 pt. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems, and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible, and shall not exceed 11 x 17 inches in size. Pages shall be numbered consecutively within each volume. Each part shall be submitted as a separately- labeled electronic file. Offerors must ensure that pricing information is only included in PART II; do not include any pricing information in PART I. All unit and extended prices provided shall be rounded to the nearest dollar. Extended prices must be divisible by the number of units proposed with no fractional cents. Submit your proposal in accordance with the table below. All electronic submissions shall reference the solicitation number in the subject line.

Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the Government’s evaluation of the proposal. Page limitations may be placed on responses to Interchange Notices (INs). The specified page limits for IN responses will be identified in the letters forwarding the INs to the Offerors.

(1) Part I: Cover Letter

Cover Letter shall include:

(a) Solicitation number,

(b) Small business status/categories,

(c) Company primary and alternate points of contact with email/phone and who is authorized to contractually obligate the company,

(d) CAGE Code, DUNS Number,

(e) A statement that the company understands the requirements specified and will meet the performance standards and requirements therein,

(f) A statement that the company does or does not take exception to any of the requirements of this solicitation,

(g) Specific information if proposing a teaming arrangement, and

(h) A statement that the quote is valid until 30 June 2021

(2) Part I: Technical Capability

Offeror shall submit the following to demonstrate their technical capability:

(a) Staffing Plan

1) Offeror shall provide a staffing plan that clearly outlines their proposed approach to ensure that they will successfully fill the required behavioral health position within 60 days.

2) Offeror shall provide a staffing plan that clearly outlines their proposed approach to ensure that they are able to fill unexpected personnel absences in accordance with the Performance Work Statement.

3) Offeror shall provide the proposed hiring timeline from the date of contract award to the performance start of the behavioral health provider. The timeline shall show the number of days from date of contract award to performance start of behavioral health provider.

4) Offeror shall provide the proposed timeline for filling the required position when unexpected personnel absences occur. The timeline shall show the number of days to fill the position from date of absence to replacement start date.

(b) Previous contract performance information

Offeror shall submit at least one (1), but no more than three (3), satisfactorily performed contract(s) in the last five (5) years that filled a behavioral health position with the same or similar qualifications as the qualifications outlined in Attachment 1 – Performance Work Statement.

The previous contract information shall include the following:

Notes:

1) Previous contracts performed more than five (5) years ago will not be considered.

2) To be considered similar or the same as the required qualifications in this requirement, previous contracts must include the following:

a. Filled a behavioral health position with a Secret Clearance

b. Filled a behavioral health position with a Doctorate degree in any behavioral health discipline.

(3) Part II: Price

Offeror shall complete Attachment 4 – Pricing Schedule of the solicitation. All unit and extended prices provided shall be rounded to the nearest dollar. Extended prices must be divisible by the number of units proposed with no fractional cents.

NEW MEXICO GROSS RECEIPTS TAX: The State of New Mexico assesses a Gross Receipts Tax for firms conducting business within state. This tax is imposed on the basis of revenue derived from business operations within the state without regard to the location of the business entity. Offeror are advised that any offer received will be presumed to include this cost of business and any request for adjustment of your prices, as a mistake in bid, either before or after contract award, will not be considered. The rate of the New Mexico Gross Receipts Tax may be obtained from the New Mexico Taxation and Revenue Department (http://www.tax.newmexico.gov) at (505) 827-0700.
