Statement of Work (SOW) – 20 March 2023
1.0 SCOPE
1.1 To provide annual preventive maintenance for the audio/visual equipment and its related components at building 148 located on MacDill Air Force Base, 2610 Pink Flamingo Ave, MacDill AFB FL 33621, for one base year and four option years.
2.0 PREVENTIVE MAINTENANCE (PM)
2.1 On-Site preventative maintenance by Authorized Service provider will:
• Clean and test equipment
• Test all routing signals
• Provide detailed report of any deficiencies and recommend corrective actions
• Provide maintenance renewals and any re-certifications needed on existing equipment
• Provide firmware and software upgrade(s) for equipment.
2.2 Preventative Maintenance Includes
• On site visits (twice yearly)
• Rapid telephone support (unlimited)
• Help desk support
• Expedited Freight
• Replacing equipment is outside the scope of the service plan; however, if the equipment is under manufacturer warranty we will facilitate the RMA with the manufacturer
3.0 Emergency Service
• Provide two emergency visits per year. Emergency visits are outside the preventive maintenance visits.
• For emergency visits we will respond on-site during business week excluding all federal holidays.
4.0 Operating Hours
Repairs will be performed during standard working hours, between 0700-1600 hours, Monday- Friday, excluding holidays and down days.
5.0 Deliverables
Provide documentation on the maintenance performed and any observations made during the maintenance. Provide the documentation in memo form (E-mail is acceptable) within seven (7) days after site visit to:

6.0 DISPOSAL OF MATERIAL The contractor will be responsible for disposing of material in compliance with all applicable environmental regulations.
7.0 GOVERNMENT/CONTRACTOR FURNISHED PROPERTY AND SERVICE:
7.1 Contractor Furnished Parts and Materials: The contractor will provide a proposal to the site POC for negotiation of the required parts and materials, identified as needing replacement or repair per annual maintenance visit.
7.2 Government Property Protection: The contractor will provide protection to government property to prevent damage during the period of time the property is under the control or possession of the contractor.
7.3 Government Property Damage: The contractor will record and report promptly to the Program Manager and designated Government Site POC, all available facts relating to each instance of accidental damage to government property or injury to either contractor or government personnel. The contractor will not perform any work on the damaged equipment/property until released by an authorized government representative. If the government elects to conduct an investigation of the accident, the contractor will cooperate fully and assist the government personnel until the investigation is completed.
8.0 GENERAL INFORMATION: Contractor Employee: The contractor will not employ persons for work on this contract if such employee is identified to the contractor by the Government Site POC as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. Contractor personnel will meet site access requirements. The contractor personnel may be required to undergo a background investigation to perform their duties at restricted sites.
9.0 SECURITY REQUIREMENTS:
9.1 The Contractor: will follow all security procedures established by MacDill AFB Security Forces and MacDill AFB Command Post at all times.
9.2 Employee List: The contractor will, following award of the project, provide a list of each employee who will require access to the base, including the employees of all subcontractors. The list must include employee name (first, middle, last), DOB, driver’s license/identification card number or other government issued photo identification card, social security number issued in the same name as the photo identification card.
9.3 Vehicles: All contractors' vehicles must have current registration and proof of insurance; the vehicle operator must have a valid driver's license. Vehicles may not remain parked at or in the vicinity of the Visitor's Reception Facility, or at any MacDill Air Force Base gate for longer than 30 minutes. Vehicles left unattended for more than 30 minutes are subject to being towed at the owner's expense. ALL VEHICLES ENTERING OR ON MACDILL AFB ARE SUBJECT TO
INSPECTION BY SECURITY FORCES PERSONNEL.
9.4 AF Form 75, Visitor/Vehicle Pass: The contractor will be responsible for the return of all AF Form 75’s issued to any employee working on the project to the 6th Contracting Squadron after completion of the contract.
9.5 Violations: Violations of these security requirements will revoke any or all passes. The Government reserves the right to deny access to any person, with no consequence to the contract.
